In the year 2046, chaos envelops the Olduvai Research Facility on the Red Planet. An unseen horror begins attacking the scientists, pulling them into the darkness and instilling panic throughout the facility.

Dr. Carmack sends a chilling transmission announcing a Level 5 security breach. As he witnesses a terrifying creature through a torn-open door, the transmission abruptly cuts off, leaving the fate of the scientists in uncertainty.

Back on Earth, the Rapid Response Tactical Squad (RRTS) prepares for a much-needed leave. However, all plans change dramatically as Sarge receives urgent orders to investigate the seismic events at the Olduvai Facility, where communication has been lost.

Despite Sarge's concerns about John 'Reaper' Grimm's emotional state, Reaper insists on joining the mission. Driven by the knowledge that his sister is among the trapped personnel, he gears up to uncover the truth behind the chaos.

The Marines are deployed to Mars using the Ark, a teleportation device discovered in 2026. Their journey is marked by the encounter with Pinky, who has undergone a horrific transformation due to a teleportation mishap.

Once on Mars, the team reunites with Dr. Samantha Grimm, who provides critical updates about the situation. She explains the connection between the humanoid remains and the monstrous beings, highlighting the existence of the 24th chromosome.

As the Marines delve deeper into the facility, they confront a deranged Dr. Carmack and face off against genetically altered creatures. The tragedies escalate, revealing the dark consequences of the experiments performed on humans.

The team suffers significant losses due to relentless attacks from the horrifying creatures roaming the facility. The remaining members grapple with the dire situation and mounting tensions within their ranks.

The situation escalates further when an Imp escapes from the Ark, wreaking havoc back on Earth. Sarge responds with his infamous weapon, the 'Big Fucking Gun,' as the Marines confront chaos at a UAC facility.

Upon arriving at the devastated UAC facility, the Marines encounter non-infected survivors amidst the chaos. The scene becomes intense as decisions are made, leading to shocking confrontations and escalating insubordination.

In a critical moment, Reaper is gravely injured but is saved by Sam, who injects him with the 24th chromosome. Instead of turning into a monster, he gains superhuman abilities, preparing him for the daunting battles ahead.

The action shifts to a first-person perspective as Reaper battles through numerous waves of enemies. He encounters grotesquely transformed Pinky, now a terrifying Demon, pushing his newfound abilities to the limit.

After obliterating Sarge and the Ark, Reaper escapes with an injured Sam, leaving the horrors of the facility behind. As they make their way back to the surface, they hold onto hope, reflecting on their sacrifices and losses.
