Alissa Vincent, the security chief, records a desperate video message announcing that the Ishimura is lost and that both the ship and the terrifying Marker must be destroyed. The story unfolds as a prelude to that grim transmission, pulling us back to the desperate days on the colony world Aegis VII.

On Aegis VII, in the second year of an illicit mining operation funded by the Church of Unitology, the colony’s geologist, Jen Barrow (/actor/lia-sargent), uncovers a monolith-like artifact known as the Marker. At first glance it seems sacred to the Unitologists, a symbol of faith and purpose, but as the days pass it becomes clear that the Marker is more than a relic. It is a human-made reproduction of an alien artifact, and its presence begins to exert a corrosive influence over the minds of those around it. The revelation sets off a chain reaction that shakes the colony’s foundations and draws the attention of the Ishimura, a distant mining ship with its own ironies and obsessions.

The Marker is brought aboard the Ishimura under the authority of Captain Mathius (/actor/jim-cummings) and Dr. Kyne (/actor/keith-szarabajka), who see it as a possible key to power or salvation, depending on whom you ask. Among the aboard crew, Unitologists such as Samuel Irons (/actor/kevin-michael-richardson) view the Marker as sacred, a beacon that justifies their faith and their pursuit. As the ship’s systems hum and the crew tries to go about its routine, the Marker’s influence quietly weaves its way through the ship, bending loyalties, inflaming paranoia, and awakening primal fears. Colin Barrow, Jen Barrow’s husband, becomes entangled in the events as the Marker’s reach stretches from the colony to the very heart of the Ishimura.

What follows is a brutal unraveling. When the planet cracks and contact with Aegis VII is lost, the colonists are shown to be completely overrun by the Necromorphs—reanimated, mutated corpses that multiply in the chaos. Barrow’s attempt to escape with her body to the Ishimura ends in tragedy as a stowaway Necromorph infects them, barely allowing a fragile foothold on the ship. Onboard, Vincent and her six-person team—one of whom is Ramirez (/actor/hal-sparks) and another Hansen (/actor/phil-morris)—plunge into a dangerous investigation of the disturbances. They encounter an escalating swarm of Necromorphs that tear through the crew, reducing their numbers and straining their nerves.

The situation grows more chaotic as Captain Mathius’s grip on reality begins to slip. Paranoia and delusions take root, forcing the bridge crew to restrain him. Dr. Kyne’s attempts to sedate the captain only hasten the captain’s downfall, and Mathius ultimately dies in a chaotic moment that leaves Kyne as the lone survivor on the run. Escape pods are launched in a panic, sealing everyone else aboard the doomed ship.

As the voyage to the bridge progresses, tragedy compounds. Hansen descends into madness and fatally slaughters Shen; Ramirez intervenes and shoots Hanson, trying to salvage what remains of the crew. In a selfless bid to protect survivors, Samuel Irons sacrifices himself to slow the Necromorph advance. Vincent and Ramirez push forward to confront Dr. Kyne, who is attempting to deactivate the ship’s engines and crash the Ishimura into Aegis VII. Ramirez dies during the arduous trek, and Kyne reveals the Marker’s overwhelming influence before escaping once more.

With the Ishimura still operational but spiraling toward a decaying orbit, Vincent reaches the Marker. It remains untouched by the Necromorphs—drawn to something in its quiet, sealed presence—allowing her to deliver one last, exhausted message to the outside world before making a final, desperate bid for escape. She launches a distress beacon and opens the bay doors, sending herself and nearby Necromorphs into the cold vacuum of space in a final bid for survival. The UGS Kellion answers the call, but the fate of Ishimura—and the Marker—remains shrouded in the silence of space, leaving the audience with the unsettling question of whether any hope remains when faith, fear, and survival collide.
