In 1956, a family gathering takes a horrific turn when an unseen assailant stabs a victim to death. The chilling scene intensifies as a bloody knife falls perilously close to a young child's feet, leaving a lasting impression of terror.

Twenty years later, in Turin, Professor Giordani hosts a parapsychology conference featuring psychic medium Helga Ulmann. During her presentation, Helga suddenly experiences a surge of dark sensations, sensing violent thoughts emanating from an audience member, which she shares with Giordani afterward.

That night, Helga is brutally murdered in her home by a black-gloved figure wielding a meat cleaver. Marcus Daly, an English jazz pianist, witnesses this gruesome act from outside her window, leaving him horrified as he encounters her mutilated body shortly after.

The following day, Marcus finds himself in the spotlight as the eyewitness to Helga's murder, with media coverage featuring a photo of him taken by reporter Gianna Brezzi. Overwhelmed by the attention, he seeks refuge in a friend's company.

While seeking comfort, Marcus encounters Carlo's unconventional mother, Martha, who exhibits an unusual fascination with him. That night, he hears a disconcerting recording of a child's song outside his door, followed by a chilling whisper threatening his life.

Haunted by guilt, Gianna decides to help Marcus uncover the mystery behind Helga's murder. They consult Professor Giordani, who connects Helga's ominous vision of the child's song to a haunted house documented in local folklore.

Marcus searches for the haunted house mentioned in the folklore book. He finds a chilling mural within, depicting a child with a bloody knife over a dead body, igniting memories of the earlier murder, hinting at a dark connection.

As Marcus continues to delve deeper, Professor Giordani falls victim to the killer in a brutal attack, distracted by an automated doll. This tragic loss intensifies Marcus's urgency to solve the mystery and protect those he cares about.

In a shocking turn, Marcus discovers a hidden corpse in the abandoned house but is knocked unconscious by an unknown assailant. He awakens outside as the structure burns, rescued just in time by Gianna, who deciphered his plea for help.

While sheltered at the caretaker's residence, Marcus notices a drawing by the caretaker’s daughter that bears a striking resemblance to the mural he found earlier. She recalls having seen it in the school archives, bringing them one step closer to the truth.

At the school, Marcus and Gianna uncover critical links in their investigation related to a schoolboy connected to the eerie drawing. However, as Gianna exits to seek police help, Marcus faces a dangerous confrontation with Carlo, his childhood friend turned suspect.

In a tense standoff, Marcus learns that Carlo's actions stem from a traumatic childhood and a desire to protect his mother, Martha. The police arrive just in time, but Carlo's fate takes a dark turn when he is fatally caught in a garbage truck.

At the hospital, Marcus discovers that Gianna survived the assault, giving him hope. As he reflects on the series of murders, he recalls seeing the killer’s reflection, leading him to deduce that Martha, Carlo's mother, is the true perpetrator.

Marcus confronts Martha, who reveals her own tragic past, including the murder of her husband. Their confrontation escalates as she attacks Marcus, resulting in a violent struggle that culminates in her demises, such as becoming entangled in an elevator.
