Within a privatized security system, the Sprawl houses around 420,000 inmates. Inside, the Death Race has been outlawed by the new warden, hired by Weyland International, who has been tasked with taking down Frankenstein, the prison’s Death Race champion and an ironclad king of the facility. The stage is set for a collision between corporate power, ruthless ambition, and the brutal will to survive.

After a Death Race victory, a prison SWAT team moves in to wipe out Frankenstein, killing several convicts before they’re cornered and overrun by the champ’s loyal inmate army. Frankenstein speaks to the warden through a body camera, vowing that he will kill every man the warden sends to kill him. The threat is clean and personal, a single-minded declaration that the fight is far from over.

A helicopter dumps a fresh batch of prisoners onto the Sprawl, each one handed a roll of silver coins to help them navigate the brutal system. The moment they land, the gang known as the Cops—led by Death Racer Johnny Law—descends, killing most of the newcomers. Two survive and refuse to surrender their coins, proving their combat prowess by dismantling the gang and moving on with their own codes.

The male inmate, Connor Gibson, is spotted by a group of women impressed by his display of skill. They escort him to a bar run by Frankenstein, where [Johnny Law] is about to be killed for failing to recover all the coins. Connor defeats two of Frankenstein’s henchmen, and Frankenstein himself confronts him personally. The bartender, Jane, connects Connor with Baltimore Bob, who witnessed the fight and can help him navigate the Sprawl.

Baltimore Bob becomes Connor’s guide, showing him around the penitentiary and introducing him to the brutal world of the Death Race. They attend a qualifying match at the Death Pit, where riders and runners test their luck against traps and rails. The other newcomer, Gipsy Rose(/actor/yennis-cheung), wins the match, signaling that the competition will be fiercer than anyone anticipated. Jane, sensing opportunity, invites Connor into her circle, and a tentative bond begins to form as he learns the rules of the Sprawl and what it means to fight for a chance at glory.

The next day, Connor declares his desire to join the Death Race. His qualifying test is a grueling footrace up a tower to claim the keys to a car, a feat he accomplishes by fighting his way through the field and defeating the Butcher in his path. Frankenstein informs his concubine Carley to bring Connor to him; Carley makes a pass at the hopeful racer in the shower, an offer Connor declines. Frankenstein hints that victory will crown him king of the Sprawl, a prize that could redefine Connor’s entire life.

Connor grows closer to Jane, and Baltimore Bob begins to build a Death Racer with him, while a female admirer in the wings asks to pilot the machine alongside him. Connor spends a night with Jane, deepening their connection and complicating the mission that has brought him here.

At dawn, the entire prison converges on Jane’s house. Frankenstein reveals that Connor is not merely a prisoner but a covert operative—Sergeant Connor Gibson—sent inside to neutralize him. The warden’s plan shifts: Connor will be allowed to compete in the Death Race, but the stakes are higher than ever.

As the Death Race starts, Frankenstein’s newest co-pilot becomes a captive Jane, used as an insurance policy against Connor. The race thins out quickly; one by one, competitors fall, leaving Connor, Frankenstein, Johnny Law, and Gipsy Rose in the running. Connor, lagging behind, is nudged by Baltimore Bob into a dramatic revival jump over a broken bridge and back into contention. Johnny Law is eliminated, and Gipsy Rose remains in third.

In the final stretch, Connor’s fuel runs dry and Frankenstein turns to seal the win. Jane begs him to spare Connor, and Gipsy Rose closes in, ramming Frankenstein and altering the outcome. Gypsy Rose reveals herself as a fellow agent, and a rescue helicopter looms on the horizon. Torn between his old life and a possible new one with Jane, Connor hesitates, but Baltimore Bob urges him to seize the moment and become Frankenstein’s shadow, the new face of the Sprawl. He dons Frankenstein’s coat and mask, seizes victory, and the race ends with him ostensibly staying in control of the Sprawl.
