Set four years after the events of the previous film, the murderous doll Chucky, Brad Dourif, arrives in a mysterious package at the home of paraplegic Nica Pierce, Fiona Dourif, and her mother, Sarah, Chantal Quesnel. Not knowing who sent the killer toy, Sarah tosses him into the trash. That night, Nica grieves the gruesome suicide of her mother, while Barb, Danielle Bisutti, her domineering sister, arrives with Barb’s husband Ian, their daughter Alice, and live-in nanny Jill, to help settle the family’s affairs. Father Frank, A Martinez, comes to counsel the family as a storm rages outside.
Alice, curious and bold, finds Chucky in the bathroom and forms an unlikely, eerie friendship with him. During dinner, Chucky secretly slips rat poison into a bowl, and as the thunderstorm intensifies, Father Frank becomes disoriented by the poison and is pinned in his car, the roof crushed away by firefighters who arrive after a car crash, unintentionally severing his head. The night’s tension deepens as the family watches old home movies; Nica notices a young Charles Lee “Chucky” Ray in one of the reels, a chilling reminder that the doll might still be alive.
As the family deals with the shock, Alice reports that Chucky has vanished. Nica traces the return address on the package, but the storm interrupts communications with on-and-off blackouts. Jill puts Alice to bed while Barb and Jill’s affair becomes a tense subtext in the house. In the kitchen, Barb enters, and Chucky is there; a series of blackouts follows as Chucky uses water to short out power, killing Jill and leaving Barb to face the horror on the stairs as Chucky’s reflection and peeling face reveal his true nature.
Nica, now trapped in a cat-and-mouse chase through the house, attempts to reach Alice on the second floor, while Ian, skeptical and protective, tries to shield her. A blackout traps them on separate floors; Chucky corners Nica with a knife, but she manages to injure him and survive long enough to escape in a wheelchair. The attic becomes a playground for the doll as he hunts Barb and then pursues Nica, opening a brutal confrontation that ends with Chucky temporarily incapacitated, only for him to return in a dark, determined twist.
The case takes a cruel turn when Chucky reveals a horrifying backstory through flashbacks: he was a friend of the family who loved Sarah, but when she rejected him, he killed her husband and abducted Sarah after the funeral. This act of abduction and the subsequent stabbing in Sarah’s stomach set in motion the events that led to Nica’s birth as a paraplegic and the chain of murderous turns that would define the franchise.
As chaos erupts, a police officer arrives and discovers Nica under duress in the elevator, where Chucky tries to finish the kill with carbon monoxide. A desperate moment follows as Nica uses a hatchet to fight back, and the killer doll is finally decapitated. Yet Chucky’s head yanks free, reattaches, and the chase continues through the house, with Chucky dragging Nica in her own wheelchair as he moves toward the balcony.
In a desperate standoff, Nica confronts the doll and pleads for answers. The doll’s taunting story backlights the night’s motive, revealing the tangled web of love, betrayal, and murder that spiraled into Nica’s fate. The electricity flickers back, and a brutal clash ends with Nica severely wounded yet alive, while the police officer’s eyes widen as Barb’s body is discovered upstairs and Chucky watches from a chair, still plotting.
Time passes, and the legal process seals Nica’s fate as she’s found guilty and sent to an asylum. As she’s wheeled out, she brandishes a grim taunt to the doll that she’s not finished. In a chilling, final tease, Tiffany, Jennifer Tilly, appears in the backseat of the officer’s car, slits his throat, and mails Chucky to his next target. The world shifts as a new chapter begins.
Back at the old home, Alice, now living with her grandmother, comes home from school and stumbles upon Chucky. He asks where his intended host lies, and with a chilling calm, he tells her the grandmother is “in the cellar.” He entices Alice to play “Hide The Soul,” a ritual that would transfer Chucky’s soul into the unsuspecting child. The camera lingers on the grandmother’s fate as she appears with a plastic bag over her head, a haunting omen as the screen cuts to black.
The after-credits sting then jumps to the doorstep of Andy Barclay, Alex Vincent, now an adult, as Chucky arrives in another package. When Andy turns away to answer a call, Chucky seizes the moment, cuts his way out of the packaging, and fixes his gaze on the old photo of Andy and his mother from childhood. Ready for a new game, Chucky faces Andy with a chilling taunt: a gloved hand reaches for a shotgun, and in a single, decisive line, Andy counters with a grim challenge, “Play with this” before firing.
