In Ohio, Lucy Vincent and her three children are brutally murdered after a birthday party. Ed Vincent is missing and believed to be the killer. Veteran homicide detective Joe Steiner is assigned to the case, but with Vincent missing, the investigation goes cold.

A few days later in California, two men are involved in a fiery wreck. One dies and the other is disfigured, later identified as Allen Devlin. The deceased's body is burned and the survivor has amnesia, leaving authorities uncertain who he is at first.

Over the next year, Allen undergoes multiple facial reconstructive surgeries and tries to remember his past with the help of nurse Chris Graham. The two grow close and fall in love as he regains his memories. Their relationship becomes a central part of his new life.

Six years pass and Allen and Chris are happily married with children. Allen is a successful realtor, while Chris stays at home to care for their family. Their domestic life contrasts with the unresolved Vincent case.

Steiner, now forced into early retirement, continues to pursue clues about the Vincent murders in his free time. His former colleague Phil Murphy informs him of a tip: a newspaper article features the Devlins and hints that Allen resembles Ed Vincent.

Steiner travels to California to confront Allen and formally expresses his suspicion that Allen may be Ed Vincent. Police Chief Michael Patterson urges him to drop the investigation and protect Chris.

Allen hires private investigator John Davey to confirm Steiner's accusation. Davey locates Theo Grant, who lent the vehicle used in the crash, but Grant refuses to talk unless compensated.

That night, Chris receives a call from a distorted voice claiming she is Lucy and accusing her of having an affair. The unnerving call intensifies the sense that the past is reaching into the present.

A woman is attacked in a park by a man wearing a leather mask, echoing the masked attacks from Ohio. Steiner deduces these assaults are tied to the Vincent murders and their cycle of violence.

Davey is found murdered. Patterson orders a search of the Devlin home, but nothing concrete is found to tie Allen to the Vincent murders.

Another attack occurs and Chris receives a disturbing call. She discovers a black leather mask and, under counsel, Allen admits himself into a psychiatric hospital, claiming innocence.

Steiner and Patterson discuss the case at Patterson's home. Steiner uncovers a mural of Chris, and Patterson admits he is still in love with her and had sent the newspaper article, though he denies making the calls. Steiner suspects the Devlin case was a manipulation to destroy Chris's marriage.

A rapist wearing a leather mask is arrested, seemingly validating the pattern of masked assaults connected to the Vincent murders. The revelation fuels Steiner's suspicions.

During a birthday party for their son, Chris is attacked by a masked assailant. She subdues him and unmasks him to reveal Allen as Ed Vincent, proving that the man killed in the wreck was the real Allen Devlin. Patterson is found dead in the home when Allen awakens, and Steiner arrives to shoot Allen to save Chris and the children.
