Annan Thampi chronicles the lives of twin brothers Appu [Mammootty] and Achu [Mammootty], two halves of a family rent apart by rivalry. Their father, Ravunni [Janardhanan], a ballet artist, makes a painful choice to separate the boys, sending Appu away to live with relatives in Pollachi while Achu stays behind. The astrologer Madhava Panicker [Jayan Cherthala] forecasts that the brothers will remain lifelong rivals, and he advises that the only way to avoid escalating conflict is to keep them apart. When Appu resists the plan, Madhava Panicker threatens him, and the moment stirs a violent response: Appu, driven by anger, tries to kill the astrologer with a rock. The plan backfires dramatically when Madhava Panicker is seriously injured and dies, and Appu is captured after Madhava Panicker’s brother-in-law intervenes. Impersonating Achu leads to a wrongful arrest, and the young man spends years paying for a crime he did not commit.
Decades pass. Appu grows into a charismatic, reckless gangster who captivates Thenmozhi [Raai Laxmi], while Achu marries Lakshmi [Gopika], building a quieter, family-centered life. Achu remains childless, and a new generation of threats looms as old enemies dig up old grievances. Thenmozhi is tragically murdered, and Achu is framed by a rival named Bharathan [Anil Murali], whose charges thrust him back into a perilous world. After his release, a devastating truth emerges: Madhava Panicker’s sister reveals that Madhav’s death was the result of his own brother-in-law’s actions, and Thenmozhi’s death was actually the consequence of an assault gone wrong by Govindan [Siddique], the man linked to their enemies. Govindan, Madhav’s son, had also been entangled in a web designed to break the brothers apart.
Realizing Govindan’s treachery, Appu confronts his ally-turned-enemy, and a brutal misunderstanding nearly tears the brothers apart again when Appu strikes Achu, thinking he has harmed Thenmozhi. The two brothers rally, uncover Govindan’s schemes, and unite to bring him down. Govindan is arrested, and the brothers return home to rebuild what they have left. In a final, poignant note, Appu returns to Pollachi and entrusts his son to Achu and Lakshmi, entrusting them to raise him as their own, sealing a fragile reconciliation and the promise of a new beginning for their family.
