The film opens with a haunting memory from Carole’s childhood, where she plays hide and seek with her friend Laura and discovers Laura dead in a garage, apparently killed by a dog. The same dog then lunges at Carole, forcing her to take shelter inside a car. This chilling echo of the past frames the story as Carole, along with fellow doctors Samir and Mathias, volunteers in a region that used to be part of Yugoslavia.

Years later, the trio returns to France after several months of relief work, but a detour caused by a roadblock changes their fate. They are seized by masked armed men, and in the chaos of a failed escape, Mathias sustains a gunshot wound to his leg. Although he receives immediate medical attention at their destination, the trio is imprisoned in a cellar. The trio, together with others found in the basement, learns there are multiple cells hidden below and that at least one other person is trapped nearby.

As days slip by, the captors supply food and occasional medical checkups, but communication stalls because they do not share a common language. The grim realization slowly dawns: the prisoners are dealing with organ dealers. Samir is suddenly taken away, and soon afterward Carole and Mathias glimpse Samir’s corpse, his eyes and other organs removed. Mathias attempts a daring escape by wrenching open a lattice window with a makeshift tool, but the attempt is thwarted; the kidnappers seal the window with a metal plate.

Carole is dragged to a makeshift surgery room where a doctor tries to remove her eyes. She fights back, freeing a hand to stab the surgeon with a scalpel. With a bolt cutter in hand, she then navigates a chamber filled with chained dogs, confronting a childhood fear along the way. In a nearby apartment above the basement, she confronts a stout woman in a brutal duel and ends up drowning the woman in a bowl.

Carole frees Mathias and a girl named Ana, and Mathias kills one of the men with the bolt cutter when a kidnapper returns. The three—Carole, Mathias, and Ana—flee into the forest as the captors unleash three dogs to pursue them. Mathias falls behind due to a gunshot wound, managing to kill one dog with a stick, but the other two overwhelm him. Carole instructs Ana to run into a cornfield, then confronts the pursuing canines herself. As the dogs close in, she spots a grenade cord and causes the explosion to erupt, killing the dogs and leaving her momentarily deaf.

The chase continues through the fields: two men close in—one with a rifle, the other having found Ana. Carole kills the first kidnapper with a rock, then, while they try to hide, the last pursuer eventually spots them. He shoots Carole in the stomach, and when he tries to finish her off, she rams a wooden stake into his foot, wrests his rifle, and shoots him in the head. In the end, Carole and Ana are rescued by two soldiers and taken away. Ana’s presence serves as a poignant reminder of Laura, tying the survivor’s journey back to the memories that opened the film.
