Mary Walsh, Brittany Murphy, a steady banker, accompanies her boyfriend Kevin Peterson to the hospital for a routine outpatient surgery. The nurse on duty explains the procedure will take exactly one hour. When Mary returns to collect Kevin, she discovers he is missing; the hospital staff can find no record of Kevin, and Detective Franklin, Jay Pickett, arrives to investigate but uncovers no immediate explanation for his absence.
Mary is escorted to the hospital’s staff psychiatrist, Dr. Markus Bensley, Peter Bogdanovich, who assesses her as mentally unstable and assigns a troubling test: she must find Kevin and prove her own sanity. An anonymous older man approaches with information and a chilling demand: a ransom of $10 million within one hour, or Kevin’s life will be at risk. Mary is forced to embezzle from her own bank to pay, a move that pulls her deeper into a carefully laid trap.
After the funds are transferred, Mary confronts Kevin and realizes the truth: Kevin is part of the gang that orchestrated the kidnapping, and Mary has been placed at the center of an elaborate scheme to steal the money with her as the lone witness. Holloway, Scott Anthony Leet, whose cell phone rings, signals that the others are waiting for confirmation that Mary has been killed. Kevin, realizing Mary is still alive, orders the others to return and finish her off. The gang attempts to run Mary down in a car chase, but she escapes through a doorway while Amanda, America Young, stays behind to draw attention away from the pursuit. Mary fights for her life, killing Cooper, a relentless gang member played by Tim Thomerson, and continues to elude her pursuers.
Detective Franklin, chasing a final lead, hurries back to the hospital and intervenes just in time. He confronts the truth head-on and shoots the armed Kevin, who falls to his death, bringing the deadly riddle to a close and leaving Mary to reckon with what she’s survived.
