During the initial outbreak of the Rage virus, Don, along with his wife Alice and four other survivors, seeks refuge in a cottage located on the fringes of London. As they huddle together, the sound of a terrified boy banging on their door captures their attention, prompting Don to let him inside. However, they soon realize that the boy has unwittingly led infected individuals to them. Desperate to protect Alice, Don urges her to leave the boy outside, but her maternal instincts compel her to keep him safe. Ultimately, feeling he has no other choice, he abandons them, escaping by boat while Alice, the boy, and the remaining survivors face what appears to be their demise.

As time goes by and the infected begin to starve, NATO forces gain control over Britain. Twenty-eight weeks later, an American military contingent led by Brigadier General Stone arrives to settle in the area. Among the newcomers are Don and Alice's children, Tammy and Andy, who were fortunate to be abroad during the outbreak. They are granted entry into District One, a fortified safe zone on the Isle of Dogs, where they are joyfully reunited with their father.

However, within mere hours of their arrival, ignoring glaring warnings, Tammy and Andy venture beyond the safety of the perimeter to retrieve nostalgic family photographs from their old home. To their shock, Andy discovers that Alice is still alive but in a delirious, semi-conscious condition. Upon being found by American soldiers, they are swiftly escorted back to District One and placed in isolation. There, Alice is subjected to testing by Scarlet, a medical officer in the U.S. Army, and it is revealed that she carries the Rage virus asymptomatically. Overcome with guilt, Don sneaks into Alice's quarantine chamber, imploring her for forgiveness. The moment they share culminates in a devastating kiss, resulting in Don’s infection. In a frenzy, he brutally kills Alice and embarks on a violent rampage.

With Alice’s children needing protection, Scarlet quickly realizes that their genetic makeup may hold the key to a potential cure. Don’s unchecked rage triggers a sequence of events, leading to chaos as American soldiers struggle to differentiate between panicking survivors and the ever-growing number of infected. An order to shoot anyone in sight adds to the bedlam, leading Sergeant Doyle to defy orders. He escapes with Scarlet, Tammy, and Andy just as U.S. Air Force planes commence a firebombing of District One. Surprisingly, Don survives the destruction and finds refuge in the ruins of abandoned London.

Doyle’s pilot friend Flynn arrives by helicopter, urging him to abandon the civilians and head to Wembley Stadium. However, defying Flynn’s orders, Doyle insists on protecting the trio. As they attempt to escape through an abandoned Volvo V70, they find themselves immobilized by released nerve gas intended for infected individuals. With American soldiers armed with flamethrowers closing in, Doyle sacrifices himself to start the car. Unfortunately, he is killed in the process. Scarlet, Tammy, and Andy make a break for the London Underground, but tragedy strikes again when Don kills Scarlet and bites Andy.

In a moment of desperation, Tammy shoots Don, ending his reign of terror. Though Andy escapes without showing symptoms, he is now a carrier of the Rage virus. Rescue comes when Flynn arrives to airlift them away from the devastation of District One, soaring over the remnants of chaos they have just escaped. However, as their journey takes them onward to France, a chilling radio transmission from the abandoned helicopter uncovers a new horror: an infected group emerges in the Paris Métro, overlooking the Eiffel Tower, indicating that the virus has now spread to Europe.
