Dr. Sailaja performed the face transplant using her deceased son Charan's face. When Charan's body was brought to her after he was killed on the same bus attack that left Satya dying, she faced a choice: perform a simple procedure or give Satya Charan's face to help him survive. She chose to transplant Charan's face because she saw Satya's strong will to live and decided to give him a second chance with her son's face.

Charan and his friend Sharath were traveling to Visakhapatnam for a wedding when they boarded the same bus as Satya and Deepthi. During the attack by Dharma's men, Charan fought back and was stabbed outside the bus. This coincidence is what led to Charan's death and subsequently gave his face to Satya.

When ACP Ashok Varma asks Ram to describe the killer of Sravan, Ram enters a psychological state where he unconsciously recreates his original face. This happens because the trauma of the bus attack and his deep desire for revenge causes this dissociative response, revealing to the audience and potentially the police that Ram is not who he claims to be.

At the end of the movie, Satya reunites with Manju, Charan's girlfriend, under the pretense of being Charan.
