Billy Halleck is a self-absorbed, overweight lawyer living in Castle Rock, Maine. He leads a comfortable life with his wife, Heidi, and their daughter, Linda, but is oblivious to the consequences of his actions.

During a local carnival, Billy accidentally runs over an elderly Gypsy woman after being distracted by his wife. This incident sets off a chain of sinister events that will change his life forever.

Billy faces a court hearing regarding the accident but walks away with no repercussions. This lack of justice enrages the woman's family, leading them to take matters into their own hands.

The woman's father, Tadzu Lempke, a powerful warlock, curses Billy in retaliation. He utters the word 'Thinner,' marking the beginning of Billy's horrifying transformation as he begins to lose weight uncontrollably.

As the curse takes effect, corrupt Judge Cary Rossington and Police Chief Duncan Hopely are also afflicted. Both men meet gruesome ends, illustrating the curse's terrifying power and further signaling the danger Billy faces.

Despite the visible effects of the curse, Billy remains in denial about the supernatural nature of his predicament. It isn't until a clinic visit confirms his rapid weight loss that he must confront the reality of the curse he is under.

Determined to lift the curse, Billy seeks out Lempke, despite his family's disbelief. His sense of urgency grows as he realizes he cannot escape the curse’s effects on his own.

At the carnival, Billy attempts to confront Lempke but is met with hostility. He is shot in the hand by Lempke's great-granddaughter, Gina, showcasing the escalating conflict between him and the Gypsy family.

Billy enlists the help of his former client, Richie Ginelli, who believes in the curse. Richie's commitment leads to dark decisions, including poisoning and threatening the Gypsies to pressure Lempke.

Richie's aggression culminates in a violent confrontation at the Gypsy camp, resulting in chaos and fatalities, including the loss of Gina's husband. This horrific event marks a turning point in the escalating feud.

In a twisted plot, Richie impersonates an FBI agent to lure Gina into a trap. This unethical maneuver escalates tensions further, leading to violent confrontations that put everyone in peril.

Billy and Richie finally meet with Lempke, who admits he cannot lift the curse. Instead, he proposes a horrific solution: transfer the curse through a cursed pie, forcing Billy to confront his moral dilemma.

Billy, conflicted but desperate, takes the cursed pie home. In a tragic twist of fate, he inadvertently allows Heidi to eat a piece, leading to her tragic demise and amplifying Billy's guilt.
