The film follows a pathological researcher who believes a microscopic parasite called The Tingler lives along the spinal cord and kills people when terror overwhelms them. To prove his theory, he subjects a deaf‑mute woman—unable to scream—to a gruesome experiment, demonstrating that only a scream can suppress the creature and spare the victim.

A pathologist, Dr. Warren Chapin, uncovers a chilling physiological oddity: when a person experiences extreme fear, a tiny parasite—what he terms a “tingler”—attaches to the spine. This creature curls up, feeds, and grows stronger as fear rises, effectively squeezing the spine if left to coil too long. The only countermeasure Chapin can identify is a powerful scream from the host, whose voice seems to cripple the tingler and release the grip on the spine.
In tandem with these unsettling findings, Oliver Higgins, Oliver ‘Ollie’ Higgins, runs a cinema that programs silent films and becomes a connecting thread to Chapin’s work. His wife, Mrs. Martha Ryerson Higgins, is deaf and mute and therefore unable to scream. A string of eerie, seemingly supernatural events unfurls in her room, culminating in her death during an autopsy, where Chapin removes a tingler from her spine and begins to suspect a larger pattern at play.
The tension escalates when the tingler, once contained, breaks free and is released into Higgins’ theater. The creature clings to a patron’s leg, prompting panicked screams that prove crucial in driving it away at least for the moment. Chapin uses this turn of events to test a bold theory: by extinguishing the lights and urging everyone to scream, the audience itself becomes part of the containment, and the tingler is forced back toward the projection room where it can be tracked and, for a time, subdued.
Yet the danger intensifies as Chapin contemplates a drastic solution. He hatches the radical notion that the tingler might be neutralized only by reinserting it into Martha’s body, the very person whose frightful death helped reveal the creature’s existence. After Chapin leaves, Higgins, who has admitted his guilt, remains alone in the room. A supernatural echo of the past repeats itself as the door slams shut and the window seals, mirroring the moment Martha was frightened to death. In a final, unsettling moment, the tingler appears to animate Martha’s body, causing her to rise and fix her gaze on her husband, while he stands terrified and unable to scream.
The closing scene returns to the theater, where Chapin’s voice speaks to the audience, offering a stark warning about fear and the tingler that may inhabit everyone:
Ladies and gentlemen, just a word of warning. If any of you are not convinced that you have a tingler of your own, the next time you are frightened in the dark… don’t scream.
