In Ohio, Margaret St. James manages to escape from a home where she was held captive. She runs toward the road and seeks help from a passing woman, revealing her severed right hand. The traumatic escape marks the beginning of the unfolding horror related to her captivity.

Police arrive at Edward Scarka's home in Ohio and confront him. Scarka surrenders nude with something behind his back, and when he raises his hand, police shoot him, revealing he is holding Margaret’s severed hand. Moments later, Scarka dies from gunfire as police respond to the situation.

In Pennsylvania, Sarah and John Bloom go to the hospital for the birth of their son, Miles. After Scarka’s death, Miles is born with blood on him, and he exhibits an unsettling calmness, refusing to cry during shots and speaking Hungarian in his sleep, hinting at a sinister background.

Miles grows up to be an oddly calm child, eight years old, with heterochromia and disturbing behaviors. He demonstrates brutal tendencies like crushing a spider and shows signs of a split personality, including speaking Hungarian threatening language and violent impulses.

Miles' behavior sparks concern when he denies playing a game after a fall and later threatens a classmate with a wrench during school. His disturbing actions prompt his parents to seek psychological help, believing he might have a split personality or other psychological issues.

Sarah and professionals review Miles’ behavior, including a recording of him speaking Hungarian, which is a rare dialect. Psychologists theorize that Miles may have a separate, dark personality, possibly connected to a past life, and provide a disturbing comparison to a boy in India claiming past life memories.

The family dog Tallulah disappears, and John finds disturbing clues, including footage from Miles' hidden camera. They discover Tallulah’s mutilated body in the basement, confirming Miles’ or Scarka’s violent tendencies and intensifying their fears about his true nature.

Sarah dreams of Miles transforming into Scarka and grabbing her. The next day, Miles confesses love but shows advanced disturbing behavior. Sarah finds Tallulah’s body and begins to suspect that Miles may be possessed by Scarka’s malevolent spirit, especially after discovering Miles’ collection of articles on Margaret.

Sarah takes Miles to a hypnotist, Arthur, who attempts to bring out Miles’ true personality. During the session, Miles appears possessed, scratches into furniture, and threatens Arthur. Sarah learns from Arthur that Miles might be haunted by Scarka’s spirit due to his unfulfilled business, relating to Margaret’s history.

Arthur deduces that Scarka’s spirit is lingering because of his unfinished business with Margaret. Sarah uncovers evidence that Scarka, a serial killer with heterochromia, died just before Miles’ birth, suggesting the malevolent spirit has returned to possess Miles.

While driving Miles to the hospital, John confesses his love for Miles, but Miles unexpectedly attacks him, cutting his seatbelt and stabbing him, causing a car accident. John is hospitalized in a coma as a result of his injuries, further isolating the family and escalating the crisis.

Sarah gives Miles sleeping pills and attempts to confront her fears by visiting Margaret’s house, pretending to be a victim. She witnesses Miles, possessed by Scarka, stabbing Margaret. In a desperate act, Sarah tries to shoot Miles, but she is shot by a neighbor as Scarka fully takes control, killing Margaret in the process.

Scarka, now fully in control of Miles’ body, is taken in by a foster family. The boy looks into a mirror and sees Scarka’s reflection staring back at him, indicating that he has permanently become the vessel for the infamous serial killer’s spirit.
