In 1984, Captain Ray Quick [Sylvester Stallone] and Colonel Ned Trent [James Woods], seasoned explosives experts for the CIA, head out on a high-stakes mission to destroy a car carrying a South American cocaine dealer. As the operation unfolds, a shocking twist emerges: the target car arrives with a young girl inside. Ray urges aborting the mission, arguing that a child’s life isn’t a risk worth taking, but Ned presses ahead, convinced the plan must go through. The blast rips through the vehicle, the dealer is killed, and the child is lost as well. The aftermath is brutal and immediate: Ray mercilessly confronts Ned, delivering a decisive beating, then reports him to his superiors and resigns from the CIA in a rage that won’t fade with time.

Years slip by, and Ray finds himself drifting into the murky margins of Miami as a freelance hit man. He answers ads placed by a mysterious woman named May Munro [Sharon Stone], whose own past sits heavy with grief—the reason she keeps reaching out to him. She is the daughter of parents murdered by Tomas Leon, the crime boss whose network stretches through the city’s darkest corners. Despite his misgivings, Ray is drawn into May’s cause, unsettled by her insistence that she will infiltrate the gang either with his help or without it. Reluctantly, he agrees to take the job, a decision that pulls him deeper into a perilous game. May inserts herself into Tomas’ world under the alias Adrian Hastings, a ruse that helps her move unseen through dangerous circles while keeping Ray on the outside as a potential shield or weapon.

Back in Tomas’ orbit, Ned Trent has shifted loyalties, now working for Joe Leon, Tomas’ powerful father and the architect of their organized crime empire. The plan escalates as the bomb squad Ned now stands alongside becomes a tool for the syndicate’s strategic killings. To ensure control, the syndicate reaches out to the local police, nudging officials to keep Ned close and usable. May plays along, maintaining a deceptive partnership with Tomas even as she quietly watches for opportunities to guide Ray back into the line of fire. It becomes clear that May’s alliance with Ned isn’t just a chance for revenge; Ned has woven her into a much larger scheme meant to pull Ray out of hiding and into a fatal trap. The CIA’s past and Tomas’ criminal present collide, revealing that May’s participation isn’t merely voluntary—it’s coercive and deeply personal.

A staged trap targeting Tomas comes to a head with May in the room, and the resulting explosion seems to erase both May and Tomas from the world. Ned arrives at Joe Leon’s side to offer condolences, and Joe spares him for the moment, hoping to use him to locate Ray and bring him to justice. Ray and Ned each believe May is dead, and a new odd signal begins to appear on a bulletin board, a coded thread that seems to lure them into a renewed confrontation. Ray senses the trap for what it is and needles Ned with a sharp, explosive volley of words, baiting him into revealing the limits of his plan.

At Adrian Hastings’ funeral, an unexpected turn reveals that May is alive. She reappears in Ray’s life, and the two hurry to the Fontainebleau Hotel, where they share a moment of intimacy before she slips away again. Ned, meanwhile, lands in the cathedral and discovers that the person in the casket isn’t May at all. A chance encounter in the hotel lobby leads May to improvise a cover story, while a henchman is dispatched to drive her away. She manages to call Ray from a bathroom, warning him of danger, and Ray rigs the hotel room to explode just as Ned’s men close in. The blast claims three of Ned’s henchmen, and May escapes once more.

Ned’s obsession with drawing Ray out leads him to keep May alive longer, hoping to force a direct confrontation. Ned listens in as Ray tries to arrange a meeting with May at a seafood restaurant, where she uses coded language to signal that it’s a trap. May is sent inside, the room is compromised, and the restaurant erupts in chaos. Ray and May flee to a warehouse where he had previously prepared a way out, and Ned tracks them using the audio recordings of their calls. The next morning, May contemplates leaving Tomas’ orbit for good and starting anew, but Ray’s resolve to end this cycle holds them together.

The hunt reaches a brutal climax as Ned arrives with a police force arrayed around the warehouse. Ray and May find themselves cornered, and in a cruel twist, Ned steps on a mine as he pursues them, triggering a massive explosion that destroys the warehouse. Ray and May escape through a hidden tunnel, unseen and unscathed, while the aftermath leaves Ned dead and his dreams of control shattered.

In the days that follow, a final act of vengeance seals the fates of those who shaped May’s painful past. Joe Leon opens a mail slot to reveal a necklace containing a photograph of May’s parents, a cruelly booby-trapped message that detonates and kills him instantly. The bomb’s blast finally cleanses the path for Ray and May, who acknowledge the weight of the losses that brought them together, and then choose to step forward into a future they will build side by side.
