What Happens in The Burdened: Full Plot Summary

The whole story of The Burdened (2023) explained, from the opening scene to the final frame: every key event, every twist and what it all adds up to. Read it as a quick recap before the sequel, to check a scene you missed, or to finally understand what really happened.

Set in 2019, this film tells the true story of a young Yemeni middle‑class couple, Israa and Ahmed, who raise their three children in Aden. Israa discovers she is pregnant again, a shock after the family had already lost their jobs in the wake of the 2015 civil war, an event that triggered a crippling economic crisis across Yemen. Unable to afford another child, the couple decides to pursue an abortion.

In a society where abortion is culturally and religiously condemned, they face significant hurdles in obtaining help. Access to medical care is hampered by doctors who refuse to assist, and Israa’s anemia makes some traditional methods unsuitable. The weight of stigma and fear presses tightly as they navigate these obstacles.

The couple turns to Israa’s close friend, Dr. Mona, who firmly refuses to help and warns Israa against terminating the pregnancy. With doctors increasingly unreachable, Ahmed tracks down traditional midwifery options that could perform the procedure at home. When Mona learns of this, she intervenes to halt the plan at a crucial moment, escalating the tension between them.

In a pivotal turn, Mona is persuaded to reconsider and ultimately decides to perform the procedure herself after witnessing Israa’s desperation. The act leaves Mona consumed by regret, and she withdraws from Israa’s attempts to contact her. Israa, in turn, experiences fear and sorrow over the strain in her relationship with Mona, while Ahmed reassures her about the importance of continuing life for the sake of their children.

The film closes on a new day in Aden, as the family heads to their children’s school. On their faces is a complex mix of temporary relief and lingering fear about what the future may hold, underscoring the emotional and moral weight of the choices they have faced.

The Burdened Plot Explained: Your Questions Answered

The questions people ask most about The Burdened (2023): what happens in the confusing scenes, why the characters do what they do, the twists, hidden details and what the ending means. Each answer contains spoilers.

Q1

Why did Israa and Ahmed decide to pursue an abortion?

They decided on abortion primarily due to severe financial hardship. After losing their jobs following the 2015 civil war, the family struggled with Yemen's crippling economic crisis. With three children already and no stable income, they believed they could not afford to raise another child. The economic pressure, combined with the uncertainty of the war-torn environment, made them feel they had no other viable option.

  • abortion decision
  • financial hardship
  • Yemen civil war
  • economic crisis
Q2

Why did doctors in Yemen refuse to help with the abortion?

Doctors refused because abortion is culturally and religiously condemned in Yemeni society. Many physicians held moral and religious objections to performing the procedure. Additionally, Israa's anemia complicated potential medical options, making some traditional methods too risky. The combination of social stigma and medical concerns left the couple with few legitimate paths forward.

  • abortion stigma
  • religious opposition
  • medical refusal
  • Yemen society
Q3

Why did Dr. Mona refuse to help Israa initially?

Dr. Mona refused because she held firm professional and moral objections to performing an abortion. As Israa's close friend, she believed terminating the pregnancy was wrong and warned them against pursuing it. Her refusal stemmed from both ethical concerns and potentially legal fears, as the procedure carried heavy social and religious consequences in Yemeni society.

  • Dr. Mona
  • moral objection
  • friend refusal
  • professional ethics
Q4

Why did Mona intervene to stop the traditional midwifery abortion?

Mona intervened because she learned about the risky, unregulated home procedure and feared for Israa's safety. Traditional midwifery methods performed outside medical facilities carried significant health risks, especially given Israa's anemia. Mona's intervention came from a place of concern for her friend's wellbeing, though it temporarily strained their relationship.

  • home abortion
  • unsafe procedure
  • medical risk
  • intervention
Q5

Why did Mona finally agree to perform the abortion herself?

Mona changed her mind after witnessing Israa's extreme desperation. Seeing her friend in such distress, with no other safe medical options available, Mona decided to take responsibility for the procedure rather than risk an even more dangerous home abortion. Her decision was driven by compassion despite her earlier moral objections.

  • desperation
  • compassion
  • moral conflict
  • reversal
Q6

Why did Mona feel regret and withdraw from Israa after the procedure?

Mona experienced intense guilt and regret immediately after performing the abortion. The moral and religious weight of her decision overwhelmed her, leading her to distance herself from Israa. She could not face the emotional consequences of what she had done, even though she had acted out of compassion. This withdrawal reflected her internal struggle rather than any anger toward Israa.

  • guilt
  • regret
  • moral conflict
  • emotional withdrawal
  • estrangement
Q7

What caused Israa's fear and sorrow after the abortion?

Israa's fear and sorrow stemmed from multiple sources: the physical and emotional trauma of the procedure, guilt over the moral implications, and profound distress over the rupture in her friendship with Mona. She felt abandoned when Mona withdrew, creating additional emotional burden during an already difficult recovery. The experience left lasting psychological scars.

  • emotional trauma
  • friendship strain
  • guilt
  • recovery
Q8

How did the 2015 civil war influence the couple's decision to seek an abortion?

The 2015 civil war triggered Yemen's devastating economic crisis, which directly caused the family's financial collapse. Both Israa and Ahmed lost their jobs, leaving them unable to support their three children, let alone another. The war-created poverty was the foundation of their impossible choice, making the abortion less about convenience and more about survival.

  • Yemen civil war
  • economic impact
  • job loss
  • survival decision
Q9

Why does the ending show the family with both relief and lingering fear?

The ending reflects their fragile situation: they resolved the immediate crisis through the abortion, but their underlying problems remain. The economic hardship, social stigma, fractured friendship with Mona, and uncertain future in war-torn Yemen all persist. Temporary relief coexists with ongoing anxiety about what comes next, capturing the bittersweet reality of their survival.

  • bittersweet ending
  • uncertain future
  • relief
  • lingering fear
Q10

Why was Israa's anemia significant in the abortion search?

Israa's anemia complicated the medical options available to her. Some traditional abortion methods posed greater risks for women with anemia, making them potentially dangerous. This medical complication further narrowed their already limited choices, pushing them toward more risky alternatives and contributing to the overall difficulty of obtaining safe medical care.

  • medical complications
  • anemia
  • health risks
  • limited options
