During a bustling summer festival, the Angel of Death, Robert Z’Dar, issues a chilling order to his Soultaker Joe Estevez: target five souls. The victims are Zack Gregg Thomsen, Zack’s friend Brad David ‘Shark’ Fralick, Tommy Chuck Williams, Brad’s girlfriend Candice, and Zack’s ex Natalie Vivian Schilling. From the outset, the fate of these lives hangs on the rapid, brutal arithmetic of the grim reaper’s design.

A precarious ride home becomes the catalyst for tragedy. After Natalie’s friend leaves her, Zack offers a ride in Brad’s car, though Brad—high on cocaine and anxious about the unexpected passenger—hesitates. At high speed, Brad swerves to avoid the Soultaker, and the car crashes. Candice dies instantly, while Natalie, Brad, Zack, and Tommy fall into comas as their souls depart their bodies. The Soultaker seizes Candice’s soul and relays a grim report to the Angel of Death, who commands him to recover the remaining souls. Miraculously, the others return to the wrecked car, unaware their bodies have been salvaged in a hospital.

The Soultaker makes his move, declaring Brad’s soul claimed, and the other would-be survivors flee in fear. They duck into a convenience store, hoping to explain what is happening, but the cashier cannot see them. A news report on the crash confuses them further, and Natalie accuses Zack of enabling Brad’s reckless behavior. They try to call for help, only to discover that the operator cannot hear them. The Soultaker returns, pressing his claim on Natalie, yet her resemblance to a past lover of the Soultaker gives him pause. Zack rescues Natalie and they escape, leaving Tommy behind in the chaos.

The Angel of Death reprimands his subordinate for failing to seize Natalie, and the Soultaker takes Tommy instead. Zack and Natalie go to her home, where Natalie introduces Zack to her mother, Anna McMillan Jean Reiner. Anna becomes an eerie presence in the scene, especially as Zack and Natalie reconcile while Anna prepares a bath. As Natalie baths, Anna watches from the doorway, blurring the line between guardian and ghoul.

A televised announcement from Natalie’s father, Mayor Grant David Fawcett, reveals that the life-support systems will be turned off at midnight. During the broadcast, the apparition of Anna morphs into the Soultaker, who offers Natalie eternal life if she stays by his side. Zack lunges at the Soultaker, but he is overpowered. Natalie tries to shoot the demon, but the weapon proves useless. When she declares she wants to be with Zack, the Soultaker hurls him from a window; Zack survives the fall.

With midnight approaching, Natalie and Zack race to the hospital in a desperate bid to reclaim their bodies. In the elevator to the afterlife, the Soultaker captures Natalie and rationalizes that he is an angel tasked with collecting souls. He tempts her with the promise that there is nothing left for her in the living world and that Zack is already dead, offering salvation if she stays. While he hunts for Natalie, Zack encounters Brad, who has become a Soultaker himself—an ironic consequence of the code that binds those who take life to a life of reaping. Brad lends Zack assistance, and together they traverse the threshold into the beyond.

Brad provides two empty “soul rings” that may enable a return to the living bodies. They locate Natalie’s body, and she steps into one ring, but remains in a coma. A crucial twist reveals that her pendant is acting as a barrier between spirit and flesh; Zack pulls her soul free in a last, risky bid to reset the cycle when the Soultaker reappears. A frantic chase rips through the hospital until Zack makes a perilous leap off the roof to escape.

Back at the edge of time, the Angel of Death confronts the Soultaker, reminding him that it is now past midnight and his assignment is incomplete. The master finally imprisons the Soultaker into a soul ring, delivering a conclusive punishment. Zack returns to his body and, with renewed resolve, saves Natalie just as she is about to fade. The two share a quiet, hopeful reunion, and Zack visits Natalie after she is discharged from the hospital, keeping faith that life can still be salvaged from the brink of death.
