Dr. Rose Cotter (/actor/sosie-bacon), a therapist grappling with the trauma of her mother’s suicide when she was just 10 years old, finds herself drawn into a chilling vortex of fear as she conducts an emergency session with a graduate student, Laura Weaver (/actor/caitlin-stasey). Laura, visibly shaken, reveals that she has been haunted by a sinister presence following the horrific suicide of her professor, Gabriel Munoz, who gruesomely took his own life with a hammer. She describes a malevolent entity that only she can see, one that can assume the guise of others, sporting an eerie smile. In a shocking turn of events, Laura suddenly slashes her own neck with a shard of a broken vase in front of Rose.

After this traumatic encounter, Rose is questioned by a detective named Joel (/actor/kyle-gallner), who also happens to be her ex-boyfriend. Still rattled by the incident, Rose recounts Laura’s claims about being cursed by a haunting smile. However, soon after, Rose begins seeing visions of Laura grinning at her in various locations, stirring a sense of dread within her. These manifestations lead Rose to investigate Gabriel’s suicide, discovering that he too exhibited strange smiling behavior just before his death.

Determined to help, Rose checks on another patient, Carl Ranken, who was previously admitted due to a manic episode. Carl smiles menacingly at her, instilling a deep sense of fear. When Rose calls for security, she realizes that his threats may have been figments of her imagination. Concerned about Rose’s mental health, her supervisor, Dr. Morgan Desai (/actor/kal-penn), places her on a week-long leave after the unsettling events surrounding Laura’s death.

With her mental state deteriorating, Rose experiences a disturbing vision of Laura at home. During this tense moment, a security operator’s call taunts her, only for the real operator to follow shortly afterward. When her fiancé, Trevor (/actor/jessie-usher), returns home, he finds Rose in a state of distress as she realizes her cat is missing after experiencing another horrifying episode.

At her nephew Jackson’s birthday party, panic ensues when the boy finds Rose’s deceased cat inside a gift. Racked with guilt and confusion, Rose falls through a glass table, further spiraling into her emotional turmoil. In desperate need of insight, Rose turns to her former therapist, Dr. Madeline Northcott (/actor/robin-weigert), only to be dismissed when she asks for medication to treat her hallucinations, which Madeline attributes to her unresolved guilt regarding her mother’s death.

As Rose continues searching for answers, she visits Victoria Munoz (/actor/judy-reyes), Gabriel’s widow, who reveals Gabriel’s unsettling sketches of a dark entity that he believed was consuming him. Unfortunately, Victoria’s disbelief and defensiveness force Rose to exit her home in frustration. Teaming up with Joel again, they delve deeper into the web of hauntings, learning about a chain of deaths linked to suicides witnessed by various individuals – a cycle wherein the only escape from the curse is committing a murder witnessed by someone else.

Decidedly fraught with dread, Rose’s reality blurs as she realizes her time may be running out. She confronts Robert Talley (/actor/rob-morgan), her only lead to breaking the curse, who chillingly reveals that the only way to escape is through killing someone else. Amidst the panic it incites within her, Rose’s fears manifest further when Madeline appears at her door in a distorted form, threatening her even more.

In a gripping turn of events, Rose drives to her childhood home where she encounters haunting visions that unearth buried resentment towards her mother. After a tumultuous confrontation with the entity in her home, Rose succeeds in setting it ablaze, only to find herself ensnared within remnants of her past. As she reconciles with her life choices amidst a spiraling psychological breakdown, the entity manifests once more, leading Rose to ultimate despair as she engages in a devastating act of self-sacrifice while smiling hauntingly.
