Simhadri grows up as an orphan in Visakhapatnam under the care of Ram Bhupal Varma and his family, forming a father‑son bond that defines his outlook on loyalty and protection. He is closely tied to Kasturi, Varma’s granddaughter, who develops feelings for him, while he quietly tends to Indu, a mentally challenged girl, visiting her once a week and entertaining her despite keeping Indu’s presence a secret from Varma. When Varma discovers that Kasturi wishes to marry Simhadri, he arranges the union, but the wedding is abruptly called off on the ceremony day when Simhadri refuses to abandon [Indu], revealing the depth of his attachment to her.

Two rival groups descend on Simhadri, tracking him to the banks of the Godavari River where he stands with Indu. The confrontation that follows shakes Varma and his family, who witness a shocking and violent turn in Simhadri’s nature. In the chaos, a second group led by [Namboothri] helps him, and they begin to call him Singamalai Anna. Indu is injured in the turmoil and, when she regains her senses, she recalls her past and stabs Simhadri with an iron pole. He is rushed to the hospital in critical condition, and [Namboothri] explains the layers of Simhadri’s hidden history.

In the past, Saraswati, Varma’s elder daughter, elopes with her lover Aravind, a Keralite, leading to heartbreak and a long separation from Varma’s world. Simhadri learns about the sadness behind Varma’s stern exterior and travels to Thiruvananthapuram to mend the broken ties. He joins a medical and spiritual therapy spa run by Saraswati and her family, disguising himself as a patient. There he encounters Indu again, Saraswati’s daughter and Varma’s first granddaughter, and slowly helps Saraswati’s family to reunite with Varma’s clan.

Tragedy strikes when Saraswati is killed by Bala Nair, a local goon who, along with Bhai Sahib, controls the Kerala mafia within the same criminal syndicate. Enraged, Simhadri takes matters into his own hands, eradicating Bala and his small gang and earning the people’s respect as Singamalai Anna. As his vigilante justice expands, Indu becomes more uneasy about the danger surrounding him, urging a return to Andhra Pradesh to escape the peril. When Aravind carries a bomb onto a moving train, Simhadri faces a heart‑wrenching choice: save the many by stopping Aravind at the cost of his own happiness, or let tragedy unfold. He chooses to shoot Aravind, a decision that leaves Indu stunned and causes her to suffer a temporary amnesia.

Presently, Simhadri survives the ordeal and reconciles with Varma and his family, while Indu slowly comes to terms with what happened. Yet the danger isn’t over; Bhai Sahib and his henchmen arrive at the hospital to finish Simhadri, and he fends them off with help from his loved ones, the police, and friends. Throughout, the story remains anchored in themes of duty, sacrifice, and the blurred line between law and vengeance, painting a portrait of a man who becomes a protector to those who cannot protect themselves, even as his own life becomes a battleground of loyalty, memory, and identity.
