Two girls driving on the road are stopped by two policemen who attempt to rape them. One girl sends a WhatsApp voice message describing their danger, and Maran, who hails from Mayiladuthurai and runs Kokkarako TV, hears it and intervenes. The rescue sets up Maran's fight against corrupt power in the town.

The MLA, frustrated with Maran's influence, hires the goon Malli to eliminate him. Malli reaches Maran's home just as Ilakkiya goes into labor and takes her to the hospital, even donating blood. Maran arrives to witness the birth of a boy, and Ilakkiya asks him to find the man who saved them so she can thank him.

Maran keeps calling and trying to arrange a meeting with Malli. He finds Malli on the verge of death after being betrayed by his associates, and Maran defeats the attacking goons. He then admits Malli to Laksha Hospitals for treatment, realizing the fates of those who helped him are entangled with larger threats.

Malli’s torturers are revealed to be acting on orders from Ashok Mithran, a prominent advocate who wants to know who stabbed him. Mithran starts sending more goons to hospitals, and Maran fights them across Laksha and Aruna Hospitals, trying to protect the victims.

Mithran deduces Malli's location and has his goons stand near the phone booth where Malli was last contacted. A girl calls Malli's number, prompting Mithran to track and order the girl’s death. Meanwhile, the MLA sends Maran's photo to the goon, escalating the danger.

Maran employs a walkie-talkie left by a hurried police officer in a restaurant, disguising himself as a police officer to lure the goons. He leads the group to the last contact point and a tense confrontation unfolds as Mithran's men close in. Maran fights to protect the people around him and penetrates the attackers’ lines.

In a flashback, Pavithra, village top of the class and a budding lawyer, publicly names cases Mithran would head. Her bold stance enrages Mithran, who uses sponsorship and coercion to try to derail her. Pavithra’s ambitions foreshadow the deadly conflict to come.

Mithran murders Pavithra at a theme park, leaving her body outside. Her grandfather is forced to claim she died from a fall from the Ferris wheel, an attempt to cover up the crime.

Pavithra’s friends disguise themselves as lawyers and attack Mithran in court, seeking justice for Pavithra. Malli appears to intervene, but he lets them go after recognizing their motive. They later contact Malli to tell him their story and enlist his help.

The friends reach out to Malli with their account, and he agrees to assist them against Mithran, even as his own associates threaten and torture him. This alliance marks Malli’s shift toward aiding the victims rather than serving the powerful.

Back in the present timeline, Maran commits to helping the girls, but the police arrest him for allegedly stealing the walkie-talkie. He is soon released when it’s revealed that the girl he saved at the start is the officer’s wife, and she vouches for him.

Mithran abducts the two girls, prompting Maran to rush to their location and stage a rescue. A brutal confrontation ensues as Maran fights Mithran’s goons to save them.

Maran defeats Mithran in combat, and the two girls finish Mithran off themselves, bringing the terror to an end. The public learns of Mithran’s crimes, while the victims gain closure for Pavithra’s murder and the hospital tortures.

The film closes with Malli meeting Maran’s sister and Maran’s girlfriend Vasuki, signaling a fragile peace and the hope of a new balance in Mayiladuthurai. Relationships among the central characters are solidified as the credits roll.
