Amidst the politically charged backdrop of the Saffron Revolution in Burma, a ruthless SPDC officer, Major Pa Tee Tint (Maung Maung Khin), leads a cruel campaign with his junta army, terrorizing small villages inhabited by the Karen ethnic community. His soldiers engage in horrific acts, including the mass slaughter of innocents, the abduction of young boys for forced conscription, and the kidnapping of women for sexual exploitation.

Twenty years after his harrowing experiences in Afghanistan, Vietnam War veteran John Rambo (Sylvester Stallone) finds himself living a quiet life in Thailand as a snake handler and boatman. His mundane existence is disrupted when a missionary doctor, Michael Burnett ([Paul Schulze]), seeks his help to transport a medical team up the Salween River into Burma for a humanitarian effort aimed at providing care to the Karen villagers. Initially hesitant, Rambo agrees to assist, primarily at the request of Michael’s fiancée, Sarah Miller (Julie Benz).

As they navigate the river, they encounter dangerous pirates who demand Sarah in exchange for safe passage, prompting Rambo to intervene with lethal force. This incident unsettles Michael, leading him to dismiss Rambo after they reach their destination, claiming they no longer require his aid. While the missionary group strives to help the villagers, tragedy strikes when Tint’s soldiers launch a brutal attack, leaving many villagers dead and abducting the missionaries.

On his return journey, Rambo destroys the pirate’s boat in retaliation but soon finds himself back at the devastated village. A pastor from the missionaries’ church approaches him in Thailand, imploring Rambo to assist in a rescue mission with a team of five foreign mercenaries. He leads them to the village where the massacre occurred but is initially rebuffed by the team’s leader, Lewis ([Graham McTavish]). They are soon joined by Myint ([Supakorn Kitsuwon]), a local Karen rebel, who guides them through the region.

As they scout the area, they haplessly witness Tint’s soldiers tormenting their captives and realize they must act. Rambo, having secretly followed the mercenaries, intervenes just in time to eliminate the soldiers using his bow and arrow, allowing the hostages to escape. Rambo decides to join forces with the mercenaries as they plan to infiltrate Tint’s camp to rescue the remaining captives.

As dawn breaks, Tint’s forces track down the group, prompting Rambo to set a trap for them using an abandoned British Tallboy bomb, which obliterates several soldiers. The confrontation escalates as Tint’s army captures nearly everyone except Rambo, Sarah, and a sniper named School Boy ([Matthew Marsden]). In a desperate act, Rambo commandeers a jeep equipped with an M2 Browning machine gun, fiercely engaging Tint’s troops and killing many of them during the ensuing chaotic firefight.

Just as the mercenaries find themselves overwhelmed, the Karen rebels, led by Myint, join the fray, turning the tide against Tint’s men. Ultimately, as defeat looms for Tint, he attempts to flee but is intercepted by Rambo, who takes swift, deadly action.

In the aftermath of the brutal conflict, Rambo returns to the United States, seeking solace and reconnection with his father in Bowie, Arizona.
