Nocebo (2022) – Full Plot Summary & Ending Explained

While Christine (played by Eva Green), a talented children’s fashion designer, engages in a phone call, an unexpected encounter occurs when a dog approaches her. This seemingly ordinary moment takes a dark turn as the dog spreads ticks onto Christine before disappearing into thin air. One particularly troublesome tick attaches itself to the nape of her neck, leading Christine into a downward spiral of debilitating illness that baffles her family.

To assist her, a woman named Diana (portrayed by Chai Fonacier) arrives at her home, claiming to be the caregiver whom Christine had hired. Both Christine’s husband, Felix ( played by Mark Strong), and their daughter, Bobs (performed by Billie Gadsdon), harbor a palpable distrust of Diana from the very start. As Diana settles into their household, she unpacks a suitcase filled with relics and folk magic ingredients that leave Felix and Bobs feeling more uneasy.

Engaging in dark rituals, Diana uses a matchbox to summon the tick that infected Christine. In a mysterious and eerie manner, she creates an altar in the fireplace of her room, utilizing the ashes in a complex ritual intended to commune with the spirit of a child. It’s during dinner when Christine experiences sudden, intense pains in her arms, leading Diana to intervene with an unusual method—hovering her hands over Christine while tickling her, effectively alleviating the pain. Diana reveals her supernatural abilities stem from a childhood encounter with a dying old woman, the last of her kind, who transferred her powers to Diana. Until this point, Diana’s abilities served as a source of income; however, the stigma of witchcraft has rendered her a social pariah.

Despite the strange but positive outcomes of Diana’s treatment, Felix remains suspicious. He confronts her in a heated interaction, demanding she keep her distance from Christine. In defiance, Diana ominously warns him to “be careful with the stairs.” This leads to a cascade of unfortunate events, starting with Felix being attacked by Bobs’ pet canary, culminating in a panic-stricken response that tragically results in the bird’s death.

As Christine and Diana bond over the funeral of the canary, Felix’s animosity towards Diana grows. Following the mysterious disappearance of Christine’s medications, Felix investigates and discovers Diana’s altar and drugs hidden beneath her bed, leading to Diana’s abrupt dismissal from their home. On her way out, Diana asks Bobs for a favor. The next day, Bobs informs Christine of Felix’s alleged frame-up, igniting a fierce argument between the parents. Felix’s escalation leads to another attack by a canary resembling Bobs’ pet, leaving him injured and hospitalized while Diana resumes her care for a weakened Christine.

In a chilling turn, Diana performs a final ritual, revealing that rather than curing Christine, she is meting out punishment for her past misdeeds. Christine sees haunting flashbacks of Diana’s life in the Philippines, where she lived a joyful existence with her husband and daughter until tragedy struck at the hands of landgrabbers and misfortune forced them into a sweatshop, operated by Christine’s clothing line. The devastating consequences of Christine’s business decisions indirectly resulted in the death of Diana’s daughter in a fire.

As Christine begs for forgiveness, Diana savagely retorts, calling her a “tick” while forcing Christine to relive the agony of that tragic fire. The horror culminates when Felix arrives to find Christine’s charred body slumped at her sewing machine. Meanwhile, Diana sends Bobs outside, instructing her to wait in the garden. In a heart-wrenching finale, Diana leaps from the roof to her demise, and as a black chick emerges from her mouth and enters Bobs’, it marks the beginning of a new, ominous chapter.

The film concludes with Bobs alone in the wilderness, foraging for herbs, while the spirit of Diana watches over her from a distance, leaving an eerie impression that their intertwined fates remain forever connected.

Ending Explained – What Happens at the End of Nocebo?

Still wondering what the ending of Nocebo (2022) really means? Here’s a spoiler-heavy breakdown of the final scene, major twists, and the deeper themes that shape the film’s conclusion.

Throughout the film, Christine’s obsession with her career and material success blinds her to the suffering of others, embodying greed and neglect. The turning point comes when Diana, a mysterious woman with supernatural powers, enters her life under the guise of helping her heal from her pain. But as Christine relies more on Diana’s remedies and spiritual practices, it becomes clear that Diana’s true motive is revenge for a tragedy that occurred years earlier. Diana’s backstory reveals that her daughter died in a fire at a sweatshop, caused by the negligence and greed of factory owners like Christine, who prioritized profits over worker safety. Consumed by her rage, Diana manipulates Christine, feeding her hallucinations of parasites and delusions of betrayal, gradually eroding her mental and physical health. In the climax, Diana’s revenge fully unfolds when she forces Christine into a ritual that links her to the fire — in this case, a symbolic and supernatural retribution for Diana’s daughter’s death. Christine is ultimately consumed in the metaphorical and literal fire at the sweatshop, as her guilt and greed burn her alive. Meanwhile, Diana, having exacted her revenge, sacrifices herself—jumping to her death after implying that she will always haunt Bobs, the daughter left behind. The ending leaves a haunting reminder of the destructive cycle of exploitation and the ghosts of past sins that refuse to stay buried. Bobs, now forever marked by Diana’s revenge, remains haunted, as the curse of guilt and revenge passes on to her, suggesting that the imprints of those wronged can linger long after the physical wounds heal. The film’s final message underscores the tragic consequences of greed and selfishness, showing how they can spawn curses that devastate generations.
