An ancient painting of Chandramukhi is distributed as a prize to Gayathri, her husband, and the Rao family in Tirupati. The gesture seems normal at first, but the painting soon becomes linked to a spate of troubling events that shadow the household over the years.

A few years after receiving the painting, Gayathri and her husband are found dead under mysterious circumstances. The family is left to cope with the loss and the growing sense that the painting is involved in it all.

Gowri, the youngest sister, is set to marry, but the wedding is aborted after a friend faints upon seeing a huge snake omen. Superstition and fear grip the family on the wedding day.

The Rao family contacts Acharya Ramchandra Siddhanthi, who enlists Dr. Vijay and his mentor Dr. Sarvanan to investigate the haunting. Early signs point to the Chandramukhi portrait as the source of the disturbances.

Investigators learn that Chandramukhi's painting resurfaced from Ganga's house, where the problem had first appeared five years earlier. The recurrence jibes with past encounters and deepens the mystery.

One night, Vijay searches the outhouse after hearing anklet sounds and discovers Gayathri alive but mentally disturbed from the past tragedy. The family admits they had been hiding her from the public eye.

Suspecting possession by Chandramukhi, the Rao family, along with Vijay and the Acharya, take Gayathri to a nearby temple for exorcism. Paranormal phenomena erupt during the ritual, but the spirit remains unconvinced to depart.

Vijay traces Chandramukhi's history to a 125-year-old tragedy involving King Nagabhairava and the dancer Chandramukhi. He learns Nagabhairava murdered her lover Gunashekaran and burned Chandramukhi, who vows revenge. The sorcerer warns that the door must never be opened, lest the spirit be freed again.

A hundred years after the original tragedy, Ganga opens Chandramukhi's sealed room and unknowingly frees her spirit. Chandramukhi flees in the form of a serpent toward Thanjavur, rekindling the danger that plagues the Rao family.

Vijay discovers that Nagabhairava is still alive, living in a cave and wielding black magic, suggesting that the threat remains potent and nearby. This discovery reframes the haunting as a centuries-spanning feud rather than a simple haunting.

Chandramukhi's spirit possesses Gowri when she reaches Chandramukhi's house in Thanjavur and mourns Gunashekaran. The Rao family realizes the true target may be Nagabhairava, not Gayathri, as Gowri sinks deeper into the legend.

Chandramukhi, manifest in Gowri, confronts Nagabhairava in a cave. A dramatic duel ends when a sword is struck by lightning, reducing Nagabhairava to ashes. Gowri is freed, and peace returns to the Rao family.
