Max's father, Jack, reopens the Mayflower Department Store in New Orleans and hires Max as a security guard. At the start, he meets the store’s manager Keller Landreaux, buyer Jenna McCarty, and VP of operations Ryan Parker. He is still haunted by his visions but accepts the job for extra stability.

Before his first shift, Max glimpses a dead woman in a mirror and then sees Jenna’s naked reflection tearing off her head. The visions foreshadow the deaths tied to the mirrors. He starts to realize he can foresee fatalities through reflections.

The following night, Max witnesses Ryan’s reflection being eviscerated and attempts to warn him, but it’s too late; Ryan dies in the same grisly manner. Jenna’s earlier death confirms the mirror’s deadly influence over the victims. Max runs to aid Ryan but arrives after the fatal moment.

Max calls his father to say he’s quitting, but Jack’s face is suddenly marked with cuts as his reflection nearly harms him. Max intervenes just in time to save Jack and resolves to stay at Mayflower. The incident solidifies the supernatural tie between their fates.

Max hints to the entity behind the mirrors that he will do whatever is asked if it spares his father. The exchange marks his growing desperation and willingness to cooperate to protect Jack. The threat establishes the bargain-like dynamic at the heart of the haunting.

Back at the Mayflower, Detectives Huston and Piccirilli recount Jenna and Ryan’s deaths and question Max’s involvement. He denies any responsibility and resumes work, but the detectives remain wary of his abilities and the strange happenings around the store. The investigation deepens the mystery surrounding the deaths.

Max goes to the main mirror to confront the ghost and the flashlight beam reveals a box with Eleanor Reigns’s ID. He finds a missing person flier for Eleanor and begins to connect the dots about her disappearance. The revelation shifts the investigation from coincidences to a murder tied to the store.

Max and Elizabeth uncover that Eleanor vanished two months ago. They learn someone deleted surveillance files the night of her disappearance, and the trail points to foul play connected to the Mayflower’s operations. The discovery implicates a cover-up at the highest levels of the store.

They seek out Henry Schow, the former guard who last accessed the files, whose mouth bears a scar from his encounter with a reflection. He reveals that Keller and the others forced him to delete the surveillance footage to conceal Eleanor’s murder. The confession ties the store’s past crimes to the present hauntings.

Eleanor, the spirit trapped in the mirror world, begins exacting revenge on those responsible for her murder. She manipulates Henry’s reflection to eat broken glass and to further influence Jenna and Ryan’s deaths. When she realizes Max can see her, she forces him to help reach her sister, threatening Jack’s life, and reveals that his near-death car accident granted him sight beyond the living.

Max and Elizabeth search the Mayflower basement for Eleanor’s body. Keller chases them and nearly strangles Elizabeth, but Max’s intervention saves her and reproduces the near-death moment, allowing Elizabeth to see Eleanor. The renewed contact confirms the haunting’s reality to both of them.

Max grapples Keller and shoves him into the main mirror. Eleanor, acknowledging Max’s help, pulls Keller into the mirror and kills him, leaving Max and Elizabeth stunned by the supernatural act. The end of Keller’s reign marks the mirror’s final vengeance.

At the police station, Henry confesses his role in the cover-up. The detectives conclude Keller is a fugitive and blame him for Jenna and Ryan’s deaths. The interrogation room mirror cracks as Eleanor appears in its reflection, and she kills Henry, sealing her vengeance as the screen cuts to black.
