Max Matheson survives a car accident that kills his fiancée, Kayla, and the crash leaves him with heavy guilt and lasting trauma. Although the other driver was held responsible, the emotional weight of the tragedy lingers, and Max leans on psychological treatment with Dr. Beaumont to regain some footing. The process is slow and the memories are vivid, but it helps him begin to reconstruct a life that was shattered in an instant.

Max’s father, Jack, reopens the Mayflower Department Store in New Orleans, and he hires Max as a security guard to replace the former guard who quit after a traumatic incident on duty. On the job, Max is introduced to the store’s inner circle: Keller Landreaux, the store manager; Jenna McCarty, a buyer; and Ryan Parker, the vice-president of operations. The environment is a glossy front for a place with its own pressures and secrets, and Max finds himself navigating a web of personalities while wrestling with his own fear and sense of responsibility.

Before his first shift, Max experiences a chilling vision: a dead woman appears in a mirror, and soon afterward Jenna’s own reflection seems to tear away at her. In a terrifying sequence, Jenna’s shower door shatters under the shock of her mirrored image, and a shard of glass slices through her, leaving a death that seems both sudden and preordained. Max realizes he has a troubling gift: the ability to foresee deaths that occur within the mirror world. The ominous pattern continues when, the following night, he witnesses Ryan’s reflection being ripped apart, and the real Ryan dies in the same manner. Max races to Ryan’s apartment, but arrives too late to prevent the tragedy.

Shaken, Max attempts to tell his father he plans to quit, only to see Jack’s face marred by cuts in a shocking vision. He rushes to Jack’s home and finds the danger is real—Jack’s reflection nearly harms him—but Max intervenes just in time to save him and decides to stay at the Mayflower, unsettled but compelled to stay close to the danger. Max senses that some force is manipulating the mirrors, and he hints that he will comply with its demands if it spares his father. He returns to the Mayflower, and detectives Huston and Piccirilli question him about the recent deaths, but Max denies involvement and is allowed to resume his duties, though the detectives remain wary.

Driven by a need to understand his visions, Max ventures to the main mirror to uncover the ghost’s identity. His reflection becomes a guide, using the flashlight he carries to lead him toward the truth. The light reveals a box containing an ID for Eleanor Reigns, a new Mayflower employee who disappeared two months earlier. A missing-person flier corroborates the disappearance. Max reaches out to Eleanor’s older sister, Elizabeth, to learn more about what happened. Together they uncover a deeper layer of wrongdoing: someone deleted surveillance files on the night Eleanor vanished, and the store’s computer records no longer show what occurred.

Max and Elizabeth seek answers from Henry Schow, the former security guard who last accessed the files. His reflection bears a scar that marks him in the same way as the others in the mirror’s world, and through Henry they uncover the truth: Eleanor is dead. Two months before, during the Mayflower’s groundbreaking party, Jenna drugged Eleanor while Ryan spoke with her, and Keller Landreaux—who raped Eleanor—killed her when she tried to flee. Henry had discovered Eleanor’s body and was forced to erase the surveillance evidence to protect Keller from exposure.

Following her death, Eleanor’s spirit—trapped within the mirror world—began a quest for revenge against those responsible for her rape and murder. She manipulates Henry’s reflection to drive Jenna and Ryan to ruin, using shards of glass as a weapon. When Eleanor realizes that Max can see her, she compels him to help forge a connection with her sister by threatening Jack’s life. Max recognizes that his near-death experience has granted him a doorway into the spirit realm, and he accepts the burden.

Max and Elizabeth return to the Mayflower to locate Eleanor’s body in the basement. Keller pursues them, keen to prevent exposure of the truth. He nearly chokes Elizabeth, but Max intervenes, triggering a replay of the near-fatal moment and enabling Elizabeth to perceive Eleanor. Upon seeing the ghost again, Max battles Keller and forces him toward the main mirror. Eleanor acknowledges Max’s help and, with a final act of power, pulls Keller into the mirror and kills him, leaving Max and Elizabeth in stunned silence.

At the police station, Henry confesses his role in the cover-up, effectively admitting guilt in Eleanor’s murder. The authorities suggest Keller has become a fugitive and blame him for Jenna’s and Ryan’s deaths, a sanction that leaves the store’s dark history on the verge of exposure. In a final, uneasy moment, the one-way interrogation room mirror begins to crack as Henry sits alone, and Eleanor appears in the reflection, signaling that survival for him is impossible. Eleanor’s figure remains, and the screen fades to black as she claims another victim.
