Madhouse

A young boy attempts to escape Cunningham Hall Mental Facility, but his bid is cut short when he is hit by a car on the grounds. The incident leaves him presumed dead and casts a long shadow over the hospital's past. It foreshadows the hidden dangers lurking inside Cunningham Hall.

Years after the 1989 incident, psychiatric intern Clark Stevens arrives at Cunningham Hall to begin his training. Nurse Betty welcomes him, hands over keys and a walkie-talkie, and introduces him to the facility's director, Dr. Albert Franks. Clark also meets other staff, including Dr. Morton and Dr. Douglas.

On his first day, patient Carl interrupts with the claim that he should not be there and argues that reform patients are kept as prisoners. His warning hints at darker truths about Cunningham Hall's practices. Franks and head nurse Hendricks respond to the disruption.

Sara guides Clark through a tour of Cunningham Hall, stopping at the basement cells nicknamed Madhouse where the most dangerous patients are kept. A patient attacks Clark with a shard of glass, but security guard Drake intervenes. Clark also notices Alice watching a window, hinting at the hospital's eerie dynamics.

That night, Clark glimpses a young boy on the hospital grounds and later witnesses Alice being physically abused by Nurse Hendricks. The visions blur the line between reality and fear as Clark grows uneasy about what is really happening inside the facility. These incidents deepen the sense of menace surrounding Cunningham Hall.

The following day, Hendricks is found dead after an attack by a cloaked figure who uses a defibrillator to kill her. The mystery of who is targeting staff and patients intensifies the hospital's peril. Clark becomes more determined to uncover the truth behind Madhouse.

Clark is assigned to investigate the Madhouse and goes to Cell #44, where a patient identifies himself as Ben London and hints that Franks could be the killer. Ben also implies something is off with the medication regime. Clark begins compiling clues and questioning staff about the patients.

Clark learns that Drake, who was seen with a patient, is accused of murder and is arrested. He also discovers that Franks has been using placebos to manipulate patient behavior and embezzling hospital funds. These revelations expose corruption at Cunningham Hall.

That night, Sara laces Clark’s coffee with a drug, knocking him unconscious. While asleep, he is haunted by Ben’s voice and visions of the little boy, and experiences a revelatory dream about the hospital's past. The dream pushes him toward a crucial truth he has been missing.

Clark awakens and, fighting the drug effects, realizes that Ben London—once a patient as a child—has been manipulating events and that Ben impersonated the deceased medical student Clark to infiltrate the hospital. The revelation reframes all prior encounters as part of Ben's plan.

Ben enters Franks' office and finds him destroying evidence of embezzlement. He reveals his identity and murders Franks with an axe. Sara witnesses the murder and flees into the Madhouse, where Ben corners and murders her as well.

In the final turn, Ben—still formally dressed—leaves Cunningham Hall and enters another unnamed psychiatric hospital. The film closes with him continuing his sinister work, suggesting the cycle of manipulation and murder will continue elsewhere.
