Katie, an ambitious model from Missouri, works as a waitress in a busy New York City diner. Seeking to update her portfolio, she eagerly responds to an advertisement for free professional photographs, setting off a chain of events that change her life.

Katie arrives at the photoshoot where she meets three Bulgarian siblings: Ivan, Nikolai, and Georgy. During the session, Georgy develops an attraction to her, but tensions rise when Katie disagrees with Ivan over a topless photograph.

After the photoshoot, Georgy invades Katie's apartment in a shocking turn of events. He binds and gags her, subjecting her to a brutal assault while her neighbor Jayson attempts to intervene, but tragically meets a fatal end.

Following the assault, Georgy force-feeds Katie ketamine, rendering her unconscious. When she regains consciousness, she finds herself in a filthy basement, handcuffed and at the mercy of Georgy and his brothers.

Finally, Katie manages to overpower Georgy, seizing her chance to escape. However, she soon discovers that she has been abducted to Sofia, Bulgaria, intensifying the gravity of her situation as she tries to find safety.

Frantically, Katie rushes to a nearby police station where she meets Detective Kiril. He directs her to Ana, who deceptively pretends to be from a rape crisis center but has sinister intentions of her own.

Instead of providing support, Ana takes Katie to her home, where she reveals her true colors as the mother of Nikolai and Georgy. In a twisted turn, she delights in Katie's suffering, subjecting her to further torment.

Katie faces unrelenting abuse at the hands of Ana and her associates, including Valko, a family friend. As the cruelty escalates, Katie finds herself further stripped of her identity and autonomy.

As Katie seeks justice, Detective Kiril and Father Dimov begin their search for her, hoping to rescue her from the darkness that continues to surround her. They fear for her safety as her retaliation escalates.

In a brutal climax, Katie confronts and tortures Nikolai and Valko, mirroring the horrors inflicted upon her. This moment of revenge starkly highlights the cycle of abuse and the darkness that consumes her captors.

In the film's haunting conclusion, Kiril intervenes just as Katie is poised to exact her final revenge. She is left with a critical choice about her future as she heads towards the U.S. embassy, unsure of what awaits her.
