The movie begins by revisiting the chilling events of its predecessor, where six strangers were lured into a perilous escape room orchestrated by a mysterious organization known as Minos. Among them, Zoey (Taylor Russell) and Ben (Logan Miller), the sole survivors, managed to escape after killing the enigmatic Game Master. Their victory was overshadowed by the realization that Minos’s operations are based in Manhattan, prompting them to vow exposing the organization fully.

Following these traumatic experiences, Zoey has been undergoing therapy to cope with her ordeal. She and Ben have attempted multiple trips to Manhattan to gather evidence, but Zoey remains paralyzed by fear—particularly flying—haunted by the horrifying sight of friends like Amanda (Deborah Ann Woll) dying during the last game. Her therapist believes she’s holding onto subconscious clues embedded in everyday objects, which leads Zoey to decide she needs to confront her fears head-on.

Driven by this resolve, Zoey persuades Ben to drive to Manhattan instead of flying. Both remain deeply haunted—Ben even has nightmares suggesting Minos has marked them, and they’ve come close to being caught at their motel, amplifying their anxiety and desperation. When they arrive at an apparently deserted alley in Manhattan—where bustling activity was reported just days earlier—they encounter a junkie who steals Zoey’s mother’s compass necklace. This theft sets off a frantic chase into the subway tunnels, where they become caught in a train car that isolates them from the outside world.

In a startling revelation, the train car is revealed to be another twisted game set up by Minos. They realize they are in a “tournament of champions,” as they uncover that they are survivors from previous Minos games. The environment is deadly—electrified metal and riddles challenge their wit and resilience—forcing them to decode messages to escape. During this deadly puzzle, they discover the message “WELCOME BACK,” which Zoey cleverly deciphers to help the group escape through a hatch in the train.

The survivors then find themselves in an elevator, exchanging stories of their past Minos experiences. They include Brianna (Indya Moore), who was trapped with influencers, and Nate (Thomas Cocquerel), a former priest tested in his faith. Their journey continues into an underground bank filled with laser-triggered traps, where they have only ten minutes to find the correct path. They navigate the deadly riddles, with Nate sacrificing himself to buy the others time. They retrieve clues from safety deposit boxes marked with the name Sonya, which leads them closer to understanding the organization’s plans.

From here, the group progresses to a beach simulation, where the scenery quickly turns hostile as sand begins swallowing everything in sight. Rachel (Holland Rhoden) is pulled under, prompting Nate to attempt a daring rescue with a raft and rope. Tragically, the rope snaps, and Nate is pulled under and lost. They also encounter eerie mannequins—one with seashells over her eyes, and another resembling a young girl—adding a disturbing atmosphere to their escape efforts. Using an alternate route through a lighthouse, Zoey, Rachel, and Brianna manage to escape, but Ben appears to fall into the sand, leaving Zoey devastated and grappling with guilt.

Outside, Rachel reveals she is immune to pain or sensation, hinting that Minos deliberately designed the games to be extraordinarily dangerous for her. As they believe they are finally free, they are led back to what seems like downtown Manhattan through a manhole cover, only to discover they are still trapped. Brianna then reveals she’s been burned by acid rain, which is about to pour down due to a ticking timer. Zoey manages to reach safety in a taxi, but Brianna and Rachel are caught in the deadly rain, suffering fatal burns.

Inside a child’s bedroom, Zoey finds a diary belonging to Sonya, the missing daughter of Sonya (a character connected to the larger conspiracy). She also encounters Amanda (Deborah Ann Woll), who is surprisingly alive and reveals that Minos kidnapped her daughter, Sonya, offering her as a bargaining chip for Zoey to help design puzzles. Meanwhile, Ben is discovered alive but trapped in a water-filled chamber, where Zoey refuses to become the new puzzle creator. With Amanda’s help, Zoey pulls an exposed gas line to burn through the metal cage, freeing Ben just in time.

The authorities then arrive to find the three survivors—along with the bodies of those who didn’t make it. Their testimonies reveal that law enforcement has been actively tracking Minos, conducting raids to dismantle the organization, and searching for Sonya. Feeling that their ordeal might finally be over, Zoey decides to return home with Ben via plane. However, during the flight, she notices strange inconsistencies, like a woman resembling her therapist with the same belongings that had previously been clues. When she overhears a voice over the intercom claiming “Minos is in control of this plane,” Zoey realizes that the nightmare isn’t truly over and that the game might still be ongoing, with their fate now in the hands of the sinister organization once again.
