As Henry Poole, portrayed by Luke Wilson, comes to terms with his terminal illness, he withdraws to a modest tract home nestled in a working-class neighborhood of Los Angeles. In a quest for isolation, he indulges in whiskey and frozen pizza, attempting to find peace. However, this tranquility is shattered by the persistent interference of Esperanza Martinez, who fervently believes that a mundane water stain on Henry’s stucco wall is a divine image of Christ. As Esperanza’s zeal grows, Henry finds himself increasingly at odds with her convictions, dismissing the stain as nothing more than an ordinary mark.

Amidst this turmoil, Henry’s life intertwines with Dawn and her shy six-year-old daughter Millie, who has been mute ever since her father deserted them a year ago. Dawn’s belief in the water stain’s miraculous essence is reignited when Millie unexpectedly begins speaking after touching the so-called holy smear. Additionally, Patience, a bespectacled cashier, also experiences a miracle, regaining her sight after coming into contact with the blemish.

As Henry discovers that Millie’s silence was triggered by overhearing his grim prognosis in a conversation with Dawn, he becomes determined to dispel the illusion surrounding the stain and restore a sense of logic to their lives. In a moment of fury, he strikes the image with an axe, inadvertently compromising the integrity of his house and causing part of it to collapse on top of him.

The film’s resolution unfolds during Henry’s recovery in the hospital. Upon awakening, he is surrounded by Esperanza, Dawn, and a now-talkative Millie, who joyously reveal that his terminal condition has miraculously reversed. Faced with this astonishing change, Henry begins to open himself up to the concepts of hope, faith, and even miracles, signifying a significant shift in his perspective on life.
