Detective Steve Carella disguises himself as a homeless man to investigate a spate of attacks against the citys homeless. HeEndures suspicion and danger while undercover, and is eventually set ablaze by two youths in an alley. He is hospitalized but recovers and returns to active duty.

A mysterious caller threatens to kill a city official unless paid a $5,000 ransom. Kling and Brown stake out the ransom drop and shadow Tony La Bresca as he takes the box containing the money. The police have placed paper in the box to trace the handoff.

The parks commissioner is shot dead, marking a brutal turn in the extortion case. A further demand for $50,000 is issued, escalating pressure on the authorities. A second box is placed at an arranged spot and police lie in wait for its recipient.

The recipient of the second box is captured, but he proves to be an opportunist who believed the box contained marijuana. The arrest reveals that the extortion operation has more legs than initially thought.

The extortion plot escalates when the deputy mayor is killed by a car bomb. The Deaf Man's network becomes the focus of the investigation as detectives search for connections to the earlier ransom and box drops.

Detective Eileen McHenry is transferred to the 87th Precinct to help solve a series of rapes. Meyer asks her to interview a witness, a mentally ill woman who claims to have been attacked by a cape-wearing man. McHenry realizes the testimony is part of a setup and the team pursues the case.

McHenry walks through a park late at night and is approached by a man who begins to attack her. She fights him off and arrests him, bringing the rapist spree to an end.

The Deaf Man, who is partially deaf, works with Buck and Ahmad to plan extorting $500,000 from Henry Jefferson. In disguise, they infiltrate the mansion's protection and plant a bomb inside. The Deaf Man then phones Jefferson to demand the money.

The detectives shadow Tony, thinking he is connected to the Deaf Man, but discover he is a petty criminal planning to rob a liquor store with his associate Schroeder. Carella and Kling are placed inside a back room of the store, ready to catch the robbers in the act.

The Deaf Man and his associates stop to buy champagne at the liquor store, drawing the robbers inside. The robbers burst in, Buck is shot dead, and Kling shoots the Deaf Man as he tries to escape while Ahmad crashes the getaway car. Kling finds evidence linking them to the mansion bombing, allowing the authorities to avert the catastrophe.

The Deaf Man flees on foot to a nearby waterfront area, where he encounters the same two youths who previously attacked Carella. They set him on fire, and he jumps into the river to escape, with no trace of him left behind.

The police celebrate the apparent conclusion of the case, but a final twist lingers as a hand emerges from the water and retrieves a hearing aid, hinting that the Deaf Man may still be alive.
