Vic Rosa, John Leguizamo, operates in the shadows of New York City’s drug trade, running a dangerous empire around a heroin brand known as Empire. His South Bronx territory sits at the center of a brittle truce among rival dealers—Hector (“Exorcist”), Tito (“Severe”), and Negro (“Dancing Queen”)—all of whom buy from the same supplier, drug lord Joanna “La Colombiana” Menendez. The tension among them simmers beneath the surface, with each move potentially sparking a larger clash that could redraw the neighborhood’s power balance.
Victor’s girlfriend Carmen, Denise Richards, invites him to a chic white-collar party hosted by her friend Trish and her boyfriend, Jack Wimmer, Peter Sarsgaard, an ambitious investment banker. The night ripples with the lure of a different life, and the encounter sets in motion a shift that pushes Vic toward a dangerous crossroads. He seeks and receives a green light from La Colombiana’s younger brother Rafael to take out the local kingpin Tito. In a brutal shootout, Tito and his young son are killed, and the violence marks Vic with a heavy moral cost. With a baby on the way, Vic contemplates going straight and starts investing money with Jack, reaping significant returns that pull him into a lifestyle that strains his relationship with Carmen.
A high-stakes proposition arrives when Jack presents Vic with an investment opportunity boasting over 300% returns, but the catch is a minimum buy-in of $4.5 million—far beyond Vic’s current funds. Desperate, Vic turns to La Colombiana, who agrees to loan him the money on one stringent condition: deliver a 500% return and quell a feud between his best friend Jimmy and the rival Hector. Vic throws himself into a tightening web of loyalties and debts, determined to stabilize his fortune and protect his growing family’s future.
Yet the feud between Jimmy and Hector spirals, and Jimmy ends up killing Hector in the heat of the street war. La Colombiana culminates her demands with an order: Vic must kill Jimmy. Instead, Vic chooses a different path—he tells Jimmy to flee town and avoid the bloodshed. The debt is temporarily managed, as Vic secures the money from Jack and hands it over, only for Jack to vanish the next day. Vic tracks Jack down, and the confrontation turns deadly when Jack and his partner Trish resist Vic’s demand for repayment; Vic shoots them both and escapes with Carmen and her family.
Together they flee to Puerto Rico, where Vic uses the little money he has left to open a bar on the island’s southern edge. The new life is fragile, built on precarious choices and fading ties to the world he left behind. The film closes on a brutal, final twist: as Vic heads to the hospital to witness the birth of his child, Rafael arrives and shoots him in the head, ending Vic’s life in a sudden, quiet moment that underscores the price of the violence he chose to live by.
