Ray Reardon, [James Spader], a successful architect, divorces his wife and goes to a gallery opening arranged by his friend Norman, hoping to meet someone new. At the event, he crosses paths with a striking young woman who accidentally spills wine on herself and hurls sharp words in his direction. The tension flickers, but the encounter ends without a lasting impression—or so it seems.

A week later, Ray encounters Lena Mathers, [Mädchen Amick], at the supermarket. She apologizes for her earlier conduct, and they decide to share a meal. What begins as awkward politeness soon deepens into a powerful attraction. They sleep together the next night, and a short time after, they marry and start a family, choosing to build a life together and become parents.

From the outside, Ray appears to have found happiness, yet shadows creep into the marriage as Lena’s past starts to unravel. Ray notices inconsistencies in Lena’s stories about her history. An assistant for one of his clients recalls Swarthmore College from a year before Lena says she attended the school, and the memory of a university president dying of a heart attack during a campus talk clashes with Lena’s version of events. When Ray repeats the president’s name, Lena treats it as if he were only a student, hinting that she cannot recall the death in a straightforward way. A woman recognizes Lena as “Sissy” from Piru, Texas, at a restaurant, but Lena insists the woman has confused her with someone else. Lena also reveals that the friends who attended their wedding were not close companions but temp agency workers Lena hired, casting doubt on the couple’s social circle.

The doubts push Ray to pry deeper. He travels to Piru and speaks with locals, who confirm that Lena’s real identity diverges from her chosen one. Lena’s parents, Mr. and Mrs. Sneeder, visit and recognize Ray, acknowledging that Lena built a new life for herself to escape poverty and an expected marriage to Buddy. Lena eventually admits that she changed her identity from Sissy Sneeder to Lena Mathers and invented a new life story to flee her past. He cleverly manipulates his friend Larry’s wife, Elaine, into telling Lena that she had made a “mistake” in her “master plan.” Elaine hints to Lena that Lena’s alliance with Larry—who has bought a house in New Zealand without Elaine’s knowledge—might be part of her escape strategy, suggesting that Lena’s ambitions extend beyond Ray’s money.

Lena returns to the mental institution to confront Ray, and on the hospital grounds, Ray enlists a fellow inmate as a distraction and lures Lena away from the supervising attendants. Lena, acknowledging that she has built a psychopathic persona around her existence, confirms her affair with an unnamed friend of Ray and asserts that she will dispose of him as she did Ray.
