In Iraq during 2010, a small squad of soldiers, led by Lt. Griggs, stumbles upon a manmade underground cave. As they explore, they encounter terrifying sights, including a river of human skulls and a horde of bats, hinting at a dark destiny ahead.

Fast forward to 2013 in New York City, Detective Ralph Sarchie is called to a scene where he attempts to save what turns out to be a deceased infant. Overwhelmed by grief, he struggles against the coroner's urging to let go of the tiny bundle, signifying the emotional toll of his work.

Sarchie and his partner, Butler, respond to a domestic disturbance call, leading them to the home of Jimmy Tratner. There, they find Jimmy’s wife severely beaten, and in the ensuing struggle, Jimmy attacks Sarchie, revealing the dark undercurrents of his psychology.

The detectives receive a shocking report from the local zoo where a woman has thrown her 2-year-old son into the lion's pit. Although the lions are not present, the boy suffers serious injuries, showcasing the dangers that lurk within their investigations.

At the precinct, Father Mendoza suggests that the woman from the zoo incident, known as Jane, may be possessed. Sarchie remains skeptical but takes Mendoza's contact information, highlighting the supernatural elements entering his investigative world.

While reviewing a case involving a deceased father seemingly calling for his daughter, Sarchie finds a disturbing link to Jane's incident at the zoo. This connection deepens his concern about the supernatural forces at play as the lines between life and death begin to blur.
