Jackie becomes curious about what's inside the thermos and decides to take matters into her own hands. Ignoring Donwynn's warnings about safety, she steals the container and retreats to the bathroom to pry it open, believing she can discover its secrets. Her decision to open it directly leads to the virus being released into the hospital environment.

Isaac had expressed fear earlier, warning that something terrible would happen if the thermos was opened.

Emma attacks Marshall after he pressures her about her test results. At this point, Emma has already been infected and is experiencing the early symptoms of the flesh-eating virus. Her aggressive behavior and biting are a result of the infection altering her mental state and making her violent, not a conscious decision.

The previous day showed Emma discovering she was pregnant while also displaying the first signs of infection with peeling skin.

Isaac makes the desperate choice to use the last pistol round on himself rather than surrender to the infection. Recognizing that turning into a zombie would mean losing his humanity completely, he chooses suicide as the only remaining option to maintain control over his own fate.

The closing shot reveals zombies attacking a camera crew documenting the carnage, confirming that the infection has spread throughout the city and is now consuming the world beyond the hospital.

The timeline notes that the undead surge continues to devour everything in its path, with the city beginning to feel the reach of the catastrophe.

The virus originated from a 1968 military experiment at Ravenside Military Hospital. It was being tested as a bacterial weapon using an infected Soviet man named Rubinsky. When the weapon test went rogue, it turned staff into zombies, prompting soldiers to shoot everyone and use explosives to seal off the facility. A medical student stole a vial of the virus, hid it in a thermos, and was shot while fleeing, dropping the container in the woods where it remained for 37 years.

The 1968 outbreak was contained through extreme measures, but the virus was never destroyed, just lost.

Marshall dies from the infection but later reanimates as a zombie. The virus has the power to bring the dead back to life in a mutated, flesh-eating form. This reanimation is part of the virus's nature, it transforms infected corpses into aggressive undead creatures that seek to spread the contagion further.

Dr. Heller stops by to study the progression of the infection before Marshall's reanimation occurs.

Ava Flores is killed by Boris during the chaotic finale. As the outbreak reaches its peak and the survivors are being overwhelmed by the growing zombie horde, Boris attacks and kills Ava Flores. Her death represents another casualty in the rapid collapse of the hospital's safety measures.

Earlier, Ava informs Donwynn that the virus is a flesh-eating illness with brutal spread potential.
