Vivianna [Candis Cayne] is in a tense moment with ranch hand Gareth [Blake Berris], when a call from her lover interrupts. He announces he wants to break things off and has already told his wife, Alice [Victoria Profeta], about the affair—a detail Vivianna hints she may know in some way. She pleads for him to stay on the line, but the call ends abruptly as a man dressed in black attacks her, leaving the ranch in a shroud of danger.

Belinda [Guinevere Turner], Alice’s sister, is dropped at the airport by Eddie [David Fumero], Alice’s husband, and it’s clear their marriage is strained. Eddie plans to head to the city for a work matter, while Belinda’s visit is meant to be a brief stop—Alice is supposed to join her sister the next day. In the meantime, four friends—Cassie [Cathy DeBuono], Taylor [Samantha Colburn], Dorri [Nayo Wallace], and Princess [Mary Jane Wells]—meet at the airport and drive to the ranch to celebrate Alice’s birthday, somewhat begrudgingly accepting that Alice will arrive a day late.

Once at the cabin, Gareth makes advances on all the women, and the dynamics among the group start to show their edges: Princess is drawn to Gareth, Cassie resents him, and the others keep their own secrets close. BJ [Andy Gala] and Minnie [Liz McGeever] arrive early and settle in, with Minnie already sensing something off about Dorri’s health and past. The group discusses Vivianna’s sudden absence, while Cassie and BJ reveal a closer relationship than the others expected—hinting that Cassie and BJ are both exploring a same-sex connection.

Over dinner, a stain on the carpet foreshadows darker events: beneath the new carpet, blood traces suggest a history of violence in this house. The conversation shifts to the house’s past, as BJ explains that 15 years earlier a mass murder occurred during a sleepover, with the killer never found and labeled the “Vanity Killer.” Tension grows as Belinda arrives and the night unfolds with flirtations and jealousy—Belinda herself admitting to a romantic link with Eddie, which stirs anger and suspicion among the group.

The next morning brings more drama as Gareth warns of an approaching storm and again flirts with Taylor, sparking jealousy in Princess. The group ventures into town for lunch, leaving Dorri behind, and returns to find Belinda’s belongings missing. Belinda’s arrival rekindles old tensions, culminating in a brutal attack in the woods: Belinda is killed by the same shadowy figure, using her high-heel sandals as the instrument of death.

As the storm rages and night falls, the group’s fragile alliances crack. Gareth returns to help and ends up in further conflict with Taylor and Princess, but the real danger remains. The revelation that Eddie has slept with multiple women within the group rocks the camp, heightening paranoia. The morning after, the friends decide to hike, piecing together more of the carnage while discovering Taylor is missing.

They eventually find Belinda’s corpse in the woods and hurry back to the house, hoping for a signal or help. Cassie decides to ride out to fetch assistance, while the others stay inside. That night, the killer claims Taylor by forcing her to swallow pearls from her necklace, confirming that there are two killers at work rather than one. The group returns to the path and finds Gareth dying, raising suspicions about Cassie.

Moments later, a bloodied Cassie is found, and BJ, in a panic, strikes her in a misguided attempt to protect the group. In a cruel twist, BJ’s own skin-damaging cream reacts violently, killing him and leaving the others to confront the truth. Dorri finally reveals herself as the orchestrator of the killings, both 15 years earlier and in the present, explaining that the girls who visited the ranch then had been cruel to her, and the ranch hand’s son helped her seek revenge. Dorri confesses she is dying and chooses to end things by taking her own life.

A detective explains to Minnie and Alice that Cassie has died and that the present murders are connected to the past. Eddie’s attempt to reconcile with Alice is rejected, and she decides to start a new chapter on her own terms, free of the tangled lives that surrounded her birthday weekend on the ranch.
