After inheriting a 12th-century castle in Italy that once belonged to a famed Duchess, John Reilly, his wife Susan, and their blind teenage daughter Rebecca travel to the estate to visit and oversee its liquidation. The trip is shaded by grief and guilt: John wrestles with the memory of their five-year-old son’s death in a drunken crash, a tragedy that also left Rebecca without sight, while Susan blames him for the fractured family. On the estate manager’s advice, they intend to stay only long enough to settle the affairs, hoping the quiet of the old house will grant them some peace. Unseen and unspoken, Giorgio Orsino—the Duchess’s son who was imprisoned and subjected to torment by his own mother—still lurks within the castle’s ancient stones, hidden away in the damp dungeon. The tension escalates when Giorgio, after killing and eating a cat, breaks off his own thumb to free himself from the manacles that bind him, and he begins to prowl through the corridors with a predatory patience. He turns his attention toward Rebecca, quietly haunting the girl’s room and creating a growing atmosphere of dread. Rebecca insists there is someone else in the house, and while John wants to believe her, Susan remains skeptical and anxious, unsure whether the danger is real or a product of their strained minds.

The Duchess’s lifelong cruelty stemmed from a bitter past, as she chained and tortured Giorgio because her husband had abandoned her and their son for John’s mother, moving to America. When the bodies of the maid and the prostitute are discovered, the police arrest John, shattering the fragile thread of the family’s attempt to endure the crisis together. Susan and Rebecca spend the night inside the castle, praying for forgiveness and for strength to survive the horror surrounding them.

Giorgio’s rampage continues as he kills two policemen and abducts Rebecca, locking her in a cell and chaining her as a captive. Yet Susan discovers Giorgio’s one vulnerability and uses it to her advantage, drawing him into a dangerous exchange that buys Rebecca precious moments of safety. Her ruse gives them a chance to mount a rescue, and together they manage to escape the clutches of the deranged housebound killer. In a desperate bid to save his family, John escapes from the police station after a brutal confrontation and races back to the castle for a final confrontation with Giorgio.

On the roof, a selfless act becomes the turning point: John sacrifices his life to pull Giorgio off the edge and end the threat once and for all. The castle’s echoes settle into a grim quiet as Susan and Rebecca cling to each other, and John’s death is mourned as a poignant act of redemption. In the aftermath, Susan and Rebecca find solace in each other and in the fragile memory of the man they loved, reaffirming their bond as they face an uncertain future.
