The Reilly family travels to Italy after inheriting a 12th‑century castle from a Duchess and arrives to begin a new life there. They plan to stay at the castle while they liquidate the estate, following the estate executor's advice. The move marks the start of their tense, uncertain time in the old residence.

Susan openly blames John for the death of their five-year-old son in a drunk-driving crash that also left their daughter Rebecca blind. The tragedy casts a long shadow over their marriage and colors their interactions from the outset. The couple's tension threads through their stay at the castle.

The Duchess's son Giorgio, kept imprisoned and tortured in the castle's dungeons, escapes by breaking off his own thumb to free himself from the manacles. With two bodies found, the police order John, Susan, and Rebecca to remain inside the castle as they investigate the missing prostitute. The family cannot leave the weather-worn estate, heightening the tension and fear within the castle. The suspicion begins to fall on Giorgio's presence in the house.

John discovers that Giorgio is his half-brother and that the Duchess was his father's first wife. The Duchess imprisoned and tortured Giorgio out of spite for her husband's abandonment. This revelation reframes the violence as a familial vendetta rooted in the castle's dark history.

Giorgio kills two policemen and then abducts Rebecca, chaining her up in his cell within the castle. The family is pushed to the brink as Giorgio asserts his control over Rebecca's fate. This marks the story's direct physical threat now focused on child and mother.

Susan learns of Giorgio's weakness and seduces him to distract him long enough to rescue Rebecca. She stabs Giorgio in the struggle, energizing the final chase. Giorgio pursues them up to the castle's roof, where the stakes become mortal.

John escapes the police station after a fight with a policeman and races back to the castle for a final confrontation with Giorgio. He sacrifices his own life to pull Giorgio off the roof and kill him, saving his family and turning tragedy into a moment of forgiveness. John dies in the act, fulfilling the tragic arc.

Susan and Rebecca survive the massacre and mourn John at his funeral. The scene ends with the prostitute's son seen with the policeman at the funeral, hinting at the ongoing consequences of the night’s violence. The family seeks some measure of peace in the castle's shadow.
