Piazza leaves prison and is immediately harassed by his old associates led by The Americano, who insist he stole $300,000. He vehemently denies the theft, even under coercion from The Americano's volatile right-hand man, Rocco. Nelly and the police commissario also suspect him, pressuring him to cooperate.

Piazza meets his blind former godfather Don Vincenzo and his loyal capo Chino. Rocco mocks Vincenzo's authority, but Chino's loyalty to his godfather remains evident.

The Americano delivers an ultimatum: return the money and resume working for him. Piazza insists he does not have it, and he does not know who does.

Paranoid about further thefts, Rocco begins killing money couriers. Piazza observes the rising danger and the pull of the unresolved case.

Piazza is sent on a $30,000 exchange at a bowling alley. A mysterious assailant in a white scarf crashes the deal, killing their client and stealing the bag.

The Americano sends Rocco and Piazza to kill those responsible, but they discover the supposed targets are Chino and Don Vincenzo. Piazza refuses to shoot his former godfather, while Rocco coldly shoots Vincenzo and Chino escapes.

The Americano has Piazza beaten for insubordination and is about to have him killed. Piazza is spared when he convincingly argues that Rocco and his crew were behind the theft.

The Americano retreats to a rural estate with his bodyguards, including Piazza, but is killed in an ambush by a vengeful Chino. Piazza then eliminates Americano's men, and Chino dies from his injuries.

Piazza travels to an abandoned church off Milan and retrieves a blue bag containing the $300,000, revealing he had stolen the money from the Americano years earlier and orchestrated everything to get him killed.

Piazza is picked up by the police for driving with an expired license and forced to go to the station for an interview. He is processed and held for questioning.

In the waiting room, Piazza runs into Rocco, who is being questioned for the shootout at the Americano's house. Rocco notices the money bag, suggests they partner, but Piazza declines and is released.

He heads to Nelly's house with the money, planning to run away with her. Nelly is with Luca, one of Rocco's crew and the man in the scarf who orchestrated the $30,000 theft. Nelly had conspired with her secret lover Lucato to take the money from Piazza for themselves.

Luca shoots Piazza, but Piazza kills Nelly with a single punch before dying. Rocco bursts in and beats Luca to death in a fit of rage; the police drag Rocco away.
