In the wake of the disastrous events caused by West's initial experiments, doctors Herbert West and Dan Cain find themselves in the midst of a violent civil war in Peru. This unlikely environment provides them with both refuge and a grim opportunity to pursue West's reanimation serum amidst the chaos.

Amidst the violent clashes, West and Cain encounter a disturbing reality—the overwhelming number of casualties presents them with grim experimental subjects. They take advantage of the situation to further their research, delving deeper into the controversial and ethical boundaries of their work.

After a harrowing encounter with enemy forces that almost costs them their lives, West and Cain manage to escape back to Arkham, Massachusetts. They reclaim their roles at Miskatonic University Hospital, ready to continue their experiments in a more familiar, albeit risky, setting.

Back at Miskatonic University, West becomes secretly obsessed with his illicit experiments in Cain's basement laboratory. Fueled by his past breakthroughs, he begins scavenging body parts from various sources, including the hospital's morgue, to create a new being.

West achieves a significant breakthrough when he discovers that his reanimation reagent can revive individual body parts. This newfound power escalates his ambition as he dreams of constructing a fully sentient being from dismembered parts, including those of the deceased.

In a dark twist, West discovers the heart of Megan Halsey, Cain's fiancée, in the morgue. Consumed by his obsession, West persuades Cain to join him on a perilous journey to incorporate Megan's heart into their project, intensifying the moral dilemmas they face.

While investigating the aftermath of the shocking Miskatonic Massacre, West and Cain inadvertently stumble upon the preserved head of Dr. Carl Hill. This discovery sets off an alarming chain of events that intertwines with their own reanimation experiments.

Pathologist Dr. Wilbur Graves uncovers a vial of West's reagent and, intrigued, he decides to reanimate Hill's severed head. This reckless act marks the beginning of a series of deadly events, revealing the consequences of tampering with life and death.

Officer Lt. Leslie Chapham begins his investigation into West and Cain following the massacre. He hints at the dark possibilities that West's experiments could be linked to the gruesome events, which puts West and Cain in a precarious position as pressure mounts.

Under duress, West is forced to confront Lt. Chapham. In a moment of panic, he kills Chapham using a lethal cloth, seeking to protect himself from the inevitable consequences of his actions. This marks a tragic turning point where West spirals further into darkness.

A dramatic confrontation erupts between the Bride and Francesca, Cain's current partner. As Cain faces a choice between the two women, the Bride, driven by jealousy, violently removes Megan's heart, leading to devastating consequences.

In a frantic bid for survival, West, Cain, and Francesca find themselves trapped in a collapsing crypt during a climactic showdown with Hill and his undead minions. As the crypt crumbles, West's past experiments come back to haunt him, leading to a chaotic end to their harrowing night.
