Rutger Hauer plays Palmer, the celebrated novelist whose bestseller Bone Daddy is a heavily fictionalized version of a string of real Chicago murders. Palmer says he gained insider knowledge during his previous job as chief medical examiner, and that intimate access informs the chilling, procedural feel of the book. The story introduces Rocky Carlson, the agent of Palmer’s work, who is attacked in his Chicago hotel room by an unseen assailant, throwing the author’s carefully crafted fiction into a dangerous, real-life suspense.

Palmer is allowed to serve as a consultant on the investigation, and he is paired with the reluctant police officer Sharon Hewlitt. This alliance sets the tone for a tense, uneasy collaboration between a writer-turned-consultant and law enforcement. The two head to a party celebrating the appointment of Peter, Bill Palmer’s son, to his father’s former position as chief medical examiner. Peter is played by Joseph Kell, which adds a personal edge to the investigators’ sense of history and legitimacy as they chase the trail of a killer who seems to be targeting those close to Palmer’s world of crime and medicine.

The case gains a second challenger in Marshall Stone, a medical examiner who explains that Franz had both professional and personal rivalries—rivalries that intensified when Palmer, in his book, framed the killer as a medical examiner. The tension between the real and the fictional intensifies as the detectives try to map the killer’s logic to Palmer’s sensationalized story. The next day, Peter’s children discover a tibia on the hood of the family car, and Palmer becomes a target once more when the Bone Daddy taunts him with another ominous warning. Palmer manages to record part of the call, heightening the sense that the killer is watching—and listening.

Hewlitt finds Rocky’s body outside her home just as Palmer arrives, an ominous reminder that the danger has followed them into the investigation. The two rush to Franz’s house, only to find him diminished by his own fall from grace, a downfall Mort Jr. blames on Bill. Peter’s house is found ransacked and empty, and the investigators stumble upon newspaper clippings about Bone Daddy along with a copy of the novel that is missing identical pages to those that wrapped the bones sent by the killer. Palmer heads to the morgue and compares the stitches on Rocky’s corpse to Peter’s work; the stitches do not match, but they do match another body. The trail narrows as Hewlitt and a specialist de-scramble the Bone Daddy’s recorded voice, which leads them to identify Marshall Stone as the killer. He takes Bill’s fictionalized crimes as a personal affront, fueling his resolve to silence those who exposed his methods.

In a tense confrontation, Stone moves to attack Peter, but Bill and Hewlitt burst in just in time to intervene. A struggle ensues, and Hewlitt shoots Stone, sending him tumbling into a deadly pit. Reinforcements arrive to seal the crime scene, leaving the investigators to reflect on how fiction and reality collided with deadly consequences. The case closes, but the scars of the investigation linger, reminding Palmer that the line between what’s written and what’s lived can blur with dangerous precision.
