The film opens with Malorie Hayes, portrayed by Sandra Bullock, urgently instructing two children known only as Boy and Girl, played by Julian Edwards and Vivien Lyra Blair, to keep their blindfolds on as they prepare for a perilous journey across the river. With a box containing her prized pet birds in tow, she guides the children along a string trail to their boat, emphasizing the critical need for caution in this dangerous new world.

Five years earlier, Malorie, an artist expecting her first child, is visited by her sister Jessica, portrayed by Sarah Paulson. Together, they watch a news segment detailing a wave of unexplained mass suicides that have rapidly spread from Siberia to Europe. Noticing Malorie’s painting, which represents a deep sense of disconnection, Jessica reassures her that motherhood will not be the same. After a light-hearted visit to the doctor with Jessica, where Dr. Lapham is played by Parminder Nagra, Malorie witnesses the outbreak’s chilling arrival in America. This horrifying event catalyzes a spree of chaos, which culminates in a tragic accident that claims Jessica’s life right before Malorie’s eyes.

In her frantic escape, Malorie is supported by Tom, a character brought to life by Trevante Rhodes, who helps her seek refuge in a house with several survivors, including Greg (BD Wong), Charlie (LilRel Howery), Felix (Machine Gun Kelly), Cheryl (Jacki Weaver), and others. The group soon learns that they face an invisible force that drives people to suicide just by looking at it. The past reveals the harrowing moments when Olympia, played by Danielle Macdonald, desperately seeks safety with the group while the stakes continue to rise.
