The film opens in Colombia along the Amazon River, where the Yaguari tribe closes in on an illegal mining camp. As the villagers force the miners out, a gigantic snake known as Boiúna emerges and attacks the miners.

A medical boat, the R.R. Hermes, makes its way toward a nearby village carrying Allison, Callum, Miguel, Raul, and Lena. Lena has been searching for her brother Jorge, who went missing somewhere in the Amazon, but she firmly believes he is still alive.

The crew stops at a bar, where Alli sees a large drawing of Boiúna on the wall and Lena explains that the snake is a guardian of the rainforest that decides who lives and dies. There they meet a group of profiteers led by Brennan Malloy, whose shy brother Shawn takes an interest in Alli.

Shawn's Death on the Dock
Alli sneaks off to untie the profiteers' boat so it can drift away, but is caught by a drunken Shawn who tries to force himself on her. When Lena intervenes, Shawn punches her, and as Lena pushes him back, he falls and fatally hits his face on the dock. Panicking, the women dump Shawn's body in the water, unaware that a local boy has witnessed everything.

After the Hermes crew departs for another village, Brennan and his men discover Shawn's corpse in the water. He finds the boy who saw the incident and learns what happened, then demands that Lena and Alli be handed over. The Regurgitated Crocodile and Lena's Past
As the crew travels on, a crocodile's corpse pops up near the boat, startling Lena and causing her to cut her leg. Miguel and Raul inspect the body and realize the animal appears to have been regurgitated. Meanwhile, Lena tells Alli how she and Jorge were protected by the Yaguari after their parents were brutally murdered for refusing to give up their land, and how she has been searching for her lost brother ever since.

After stopping their boat somewhere dark for the night, the crew is caught by Brennan and his men, who hold the men hostage. Boiúna abruptly emerges, snatching one of Brennan's men, Miguel, and Raul, while Callum is accidentally shot in the chaos. The snake then uses her strength to capsize the boat, sending Alli and Lena into the water.

Lena arrives at a construction site where Boiúna attempts to attack her. She hides inside a crane machine and uses the crane to try picking the snake up, but a piece of equipment gets impaled into her leg, leaving her trapped and unable to escape.

Brennan returns with a chainsaw and partially slices into Boiúna, giving Lena a chance to fight back. The snake devours Brennan, but Lena manages to stab it in the side as Jorge and the other Yaguari arrive to watch.
