Maek is hired to kidnap a police informant named Iht and bring him to the mob hit house. He realizes Iht isn't bad and refuses to kill him, turning on the enforcers instead. A gun battle erupts, Maek is wounded, and Iht seizes Maek's weapon to help them escape.

Maek and Iht flee the mobsters' stronghold and ride away on a motorcycle. Iht tends Maek's wounds, and they hole up on Maek's rooftop hide-out. The two men grow closer as the danger fades, setting the stage for a budding attraction.

During a quiet moment, Iht tends to Maek in a bath and kisses him, sparking a passionate, secret affair. They exchange tormented looks as they realize their feelings run against their world of violence. The bond between them deepens as days pass.

The next day Maek orders Iht to leave him alone, and Iht reluctantly returns to his fiancée Sai. Still, his heart remains with Maek as he begins to reminisce and search for his lover in Bangkok and within Maek's world.

Iht learns of Maek's brother Mhok and their mother, who are dealing with HIV and AIDS. Mhok is HIV positive due to sexual abuse by their stepfather, and their mother is dying of AIDS. The two brothers' struggle adds a bittersweet burden to Iht's conflicted loyalties.

Maek secretly visits his mother and brother, and Iht corners him at the building entrance to declare his love. They share a kiss, a moment witnessed by Sai who remains unaware of the full truth. The emotional stakes tighten as Maek's world pulls him away.

Maek's mother overhears that Mhok prostituted himself to survive after contracting AIDS, and she commits suicide by hanging. As her body is being rushed out, a sniper's shot strikes and kills her, a bullet presumably aimed at Maek. The tragedy hardens Maek's resolve.

Maek goes after his former mobster employers and succeeds in killing them, exacting deadly revenge. The pursuit signals a shift from avoidance to direct confrontation as Iht learns of Maek's actions and the cost of their past.

Iht travels to the capo's house to stop Maek but arrives too late. In a bid to slow him down, the capo's wife shoots at a clock, shattering it into Iht's face and injuring him. The lovers' collision course grows more dangerous.

Maek meets his brother at a railway station intending to leave Bangkok for good, but police apprehend him before he can board. The moment leaves Mhok broken and the mob bosses doubling down on their pursuit.

Years pass and Iht visits Maek in prison, learning that Iht has been blinded in the final gunbattle with Maek's ex boss. Meanwhile, Mhok commits suicide in a hospice in Lopburi after losing the will to fight his disease. The double tragedy sets the stage for the lovers' long separation.

Maek is released from prison and is reunited with Iht, but their hopes are cut short when Maek is shot dead by an unseen assassin. Iht, now blind, collapses over his lover's corpse, whispering his love once more. The violence ends their chance for happiness, leaving Iht to mourn the loss.

Iht eventually regains his sight and discovers a video Maek recorded years earlier in which he admits he loved Iht all along. The final revelation lets him hear Maek's truth after his death and gives him a lasting memory to hold.
