In the end, Baghead reveals itself to be a powerful and malicious entity that has been manipulated and contained through a centuries-old cult called the Brotherhood. The creature, which can channel the spirits of loved ones by consuming their belongings, has been used as a vessel by this cult to trap spirits and keep them for dark purposes. Iris, the protagonist, gets drawn deeper into the entity’s influence, especially after Neil manipulates her by promising money and repeatedly summoning spirits from her past. Her visions grow more intense, revealing her father’s death and her own transformation into Baghead, which signifies her loss of innocence and control. Neil’s selfishness and the desire to revive his wife, Sarah, ultimately lead him to kill Iris, as he sacrifices her to free himself from the curse by becoming Baghead’s new guardian.

However, Neil’s plan backfires when he neglects to acquire the special wooden quill, the last link to signing over the deed and becoming Baghead’s true guardian. As a result, Baghead’s spirit takes possession of Iris’ body, turning her into her own guardian and amplifying her powers with the spirits of past victims, including Regina, Catherine, and Iris herself. Now in control, Baghead uses its newfound strength to torment Neil and other men who abuse women, even going so far as to murder Neil and burn down The Queen’s Head, effectively ending Neil’s threats and releasing the pub from the curse. The film’s final moments leave the audience with a sense that Baghead, now free, will continue to thrive by feeding on grief and memories, suggesting that evil spirits like her always find ways to survive and escalate.

This ending echoes themes of grief, power, and revenge, showing how the spirits of the dead—especially those twisted by trauma—can become vehicles for vengeance when manipulated by dark forces. The film emphasizes the danger of clinging to the past and of trusting false promises, illustrating that true control over such evil entities is nearly impossible. The conclusion implies that Baghead’s victory is bittersweet but fitting, as she ultimately becomes a powerful guardian of her own dark destiny—defiantly reclaiming power from those who would exploit the dead and the living alike.
