During World War II, Kurt Gerstein is introduced as a Waffen-SS officer working at the SS Hygiene Institute. He designs programs for water purification and vermin destruction, initially focused on combating typhus. His work puts him in contact with the Nazi extermination efforts, which later shock him to his core.

Gerstein discovers that the hydrogen cyanide mixture he helped develop, Zyklon B, is now being used to murder Jews and other perceived undesirables in extermination camps. This revelation horrifies him as he realizes his work is being weaponized for mass genocide.

Gerstein tries to notify Pope Pius XII about the atrocities, hoping to garner support or intervention. However, he is met with a lack of response and widespread silence from the Vatican hierarchy, leaving him feeling powerless and disturbed.

Riccardo Fontana, a young Jesuit priest, is moved by Gerstein's revelations and joins efforts to raise awareness about the ongoing mass exterminations. Despite their combined efforts, official condemnation from the Church remains weak and vague.

Gerstein travels to Rome with hopes of speaking directly to the Pope about the Nazi atrocities. His attempts are thwarted as he is not granted an audience, underscoring the Vatican's reluctance to confront the Nazi regime openly.

As Germans take control of Rome, they start rounding up Italian Jews for deportation. Fontana pleads with the Pope to intervene by protesting at the train station, but the Pope refuses, citing concerns for Christian safety under Nazi rule.

Fontana, in a symbolic act of defiance, wears the Jewish Star of David and presents himself at the deportation train. He is detained at the concentration camp, interrogated, but is allowed to stay to help the Jews working in the crematorium before being eventually gassed.

Gerstein attempts to intervene and save Fontana from being gassed, but Fontana refuses to leave his post among the Jews. Gerstein watches helplessly as Fontana is eventually killed, witnessing the ongoing massacre.

Gerstein collects extensive evidence of the Nazi atrocities, including photographs and documents. He delivers this evidence to the Allied forces in an effort to expose the Holocaust and hold the Nazis accountable.

Despite providing critical evidence, Gerstein is arrested by the Allies and is eventually found dead in his jail cell, having hanged himself. His death marks a tragic end to a man who tried to reveal the truth of the Holocaust.

The Doctor, a friend of Kurt Gerstein, approaches a Cardinal to seek help leaving Germany. Claiming to be just a physician, he is granted assistance, and promises are made to help him escape to Argentina, highlighting the ongoing chaos and attempts to flee Nazi Germany.
