In front of a church, a 1980 flashback shows Thomas Kutty losing money to a scam artist. The other three boys—Mahadevan, Govindan Kutty, and Appukuttan—rush to his aid and rescue him from further trouble. They vow never to cheat each other, sealing their bond with the line 'Thomas Kutty Vittodaa!'

Twenty years later, the friends are living separate lives. Mahadevan is a Dubai-based businessman and part-time trainer, married to Sulochana with a daughter named Meenakshi, though family life is rocky. Govindan Kutty is a successful builder, happily married to Parvathi, while Appukuttan practices dentistry in Mumbai with wife Janaki and twin sons Manu and Vinu.

The prologue follows the four as they travel back to Harihar Nagar to attend Thomas Kutty's wedding. Thomas plans to marry an orphan named Jaseentha from the same church, fueling the sense that nothing has truly changed. The friends arrive with a mix of nostalgia and trepidation about their upcoming reunion.

After settling in Govindan Kutty's old house, Mahadevan proposes they recapture their youth and return to their laugh-filled Harihar Nagar days. The group agrees to act younger and carefree, attempting a harmless act to kickstart the mood. They cause a small hotel mishap and escape.

They receive flowers at Govindan Kutty's doorstep with a tag reading 'Maya' and hope she is their old friend. They rush to meet her in a cafe but miss her, triggering a tense chase when Appukuttan throws a rock and breaks her car window. An anonymous woman steps out, later revealed as another Maya, triggering further confusion.

Their misadventure leads to trouble as they are arrested for a car-window incident and mistaken identity. Maya comes to their aid later, posting bail and asking them not to bother her again. The episode marks Maya's deepening involvement in their lives.

That night Appukuttan glimpses a ghostly figure with a burned face at the window, adding a supernatural chill to their plan of mischief. The others try to reassure him, but the mystery surrounding Maya only thickens. The eerie sight foreshadows the danger ahead.

The next morning Thomas arrives back and Mahadevan spins a sentimental tale to convince Maya they are harmless. Maya later visits them and gradually befriends the group, suggesting she might be more than an obstacle to their plans. The bond begins to shift as trust forms.

It is revealed that Maya is Cristina, John Honai's daughter, working with her evil brother Freddy. Thomas Kutty is kidnapped days before his wedding and the foursome becomes prime suspects. Freddy's men trail them, and they learn the plot centers on the mysterious box and the old 'Maya'.

Freddy forces them to retrieve a box of money from a bank and each of the four holds two digits of the password. They manage a bold escape, but Appukuttan is captured in the process. Maya emerges with crucial truths while the plan begins to unravel as the password scheme falters.

Mahadevan, Govindan Kutty, and Thomas decide to retrieve the box to pay Freddy and save Appukuttan, but the password turns out faulty, stalling their rescue. The trio confront Freddy further, buying time while the mystery deepens. The plan's flaws ramp up the tension toward the inevitable showdown.

A time bomb strapped to Freddy's pet lizard travels through the fight, catching everyone except Mahadevan. In a frantic finale, the bomb attaches to Thomas Kutty just as Freddy triggers it, killing them both. The twist reveals Thomas Kutty is alive all along and had been betraying his friends.

The three friends realize the betrayal and overpower Thomas Kutty before leaving him at the church where their friendship began. They confront the truth of the plan and decide to move on together, shaking off the deception. The moment underscores the fragility of trust but the strength of their bond.

Thomas Kutty apologizes, and the friends reconcile at his wedding, reaffirming their bond as best friends. The resolution smooths over the earlier violence and restores the group's camaraderie. The film ends on a note of enduring friendship.
