In Brooklyn, ca. 1976, Angie resists abandoning her baby Patty on a cathedral doorstep. She chooses to raise Patty on her own, surviving through wit and grit on the streets. Their early life is defined by a stubborn mother-daughter bond.

As Patty comes of age, she meets and marries the kind policeman Dante Montanari. The couple builds a family and has two sons, Joey and Nino. Their life is marked by love and the looming shadow of crime in their neighborhood.

Dante dies of cancer, leaving the Montanari family with mounting hospital bills. Patty must figure out how to keep their family afloat without her husband. The financial pressure seeds the turn toward the crime world.

Patty befriends funeral director Frank Russo and his boss Sal Palmeri, drawn by their influence in Brooklyn. Frank offers her a job running a faltering restaurant, which she turns into a thriving transvestite nightclub. This marks her entry into the crime world.

Sal Palmeri is impressed by her savvy and assigns her to run a money pickup. She travels to Toronto to retrieve a half-million dollars from a shipboard casino and delivers it to Sal, cementing her role as his courier.

Later, Patty and Sal's girlfriend June make the same trip to Canada together. En route, Patty witnesses June snorting cocaine, while back in the casino area, police raid the shipboard casino. Patty slips away and returns to the hotel, narrowly avoiding capture.

Back in Brooklyn, Frank's teenage son Mouse is selling cocaine on the streets and struggles under his father's demands. Mouse is murdered, shocking the family and the neighborhood.

During Mouse's funeral, FBI agent Robert Wilford approaches Patty, asking for help to take down Palmeri's empire. He suggests that Patty wear a wire to record incriminating evidence. The looming sting sets the stage for a dangerous confrontation.

Patty learns that June turned Mouse onto selling drugs, and that Palmeri ordered a hit on him for the street deal. She also discovers that Frank asked his boss for permission to mercifully kill his own son.

June escapes, but Patty intercepts her at her apartment and confronts the danger they face. Patty realizes that she, June, and Frank may be Palmeri's next targets, heightening the sense of looming violence. The stakes keep rising as the family draws closer to the brink.

Patty persuades Palmeri to let her go free and to spare Frank unless he continues threatening her family. She uses this leverage to turn the tables on the boss and protect her loved ones.

Palmeri and his men are arrested or killed, and June's body is discovered, presumably from an overdose. The criminal empire crumbles, and Patty's risk pays off as her family is finally safe.

The Montanaris and Angie hop on the next train to Florida, leaving Brooklyn behind. They seek to start anew away from crime, stepping into an uncertain future.
