In the town of St. Rosa’s Village, the sprawling factory owned by the megacorp BTH sits as the beating heart of a community that’s grown wary of the company’s reach. Here, a man named Mel Ferd is visited in his office by his employee Kissy Sturnevan. She has gathered hard evidence against BTH and plans to expose the scheme, knowing that powerful enemies will come after her. The scene is shattered when a gang of rapping thugs, the Killer Nutz, storm the room. Mel Ferd taunts them as they shoot him repeatedly, only for him to rise again and again—until one brutal harpoon blast finally ends him. J.J. Doherty, the tenacious associate who has been chasing the truth, flees in terror after witnessing her mentor’s death.

Behind the company’s polished public face lurks something far darker. Bob Garbinger runs BTH with his assistant Kissy Sturnevan, and the corporation’s business model hinges on more than profits—it thrives on a web of gangsters and dirty deals. The sludge they dump into the city has sickened workers and customers alike, and Garbinger’s brother, Fritz Garbinger, handles the illicit side of the operation, coordinating bets with criminal elements to keep the machine running. The danger J.J. poses forces Fritz to unleash the Killer Nutz after her, tightening the noose around anyone who would dare reveal the truth.

Meanwhile, in a nearby corner of the plant, Winston Gooze works as a janitor, a widowed stepfather to Wade, a shy boy with big dreams but a crippling stage fright that sabotages his school audition. Winston discovers a grim diagnosis—a rapidly advancing brain tumor—and the insurance system proves useless against his prognosis. He clings to a stubborn hope, attempting to cheer Wade by dancing with him in a playful moment, but Wade remains distant, preoccupied with his own fears and family’s secrets.

A dinner with Garbinger and Kissy, attended by public figures, exposes the cruelty behind the smiling facade. The crowd sees him as a savior, and Wade recognizes his stepfather’s voice on the television through the raucous din of the crowd.

Garbinger quickly detects that Toxie is no ordinary vigilante. He learns that Toxie is Winston, the man who once walked away from Wade during a difficult moment—and the revelation stings. Fritz shares the news with his brother, hinting that Wade is involved. J.J. tracks Winston’s movements and eventually finds Wade, learning that his father figure is alive in a way Wade never imagined. The Killer Nutz arrive with the explicit aim of kidnapping Wade, and J.J. fights to protect him, guiding them toward the woods where Winston’s mutating powers could still be controlled—or unleashed.

Toxie’s blood runs hot with desperation, and a lifeline arrives in the form of a makeshift doctor in the woods, a figure Guthrie Stockins, a weary hobo who becomes an unlikely ally. Using his regenerative biology, Toxie heals J.J.’s gunshot wound by means of his own blood, a reminder that the mutated hero carries a shocking medical secret with him. The two attempt to mount a rescue of Wade, but their plan is interrupted when the Killer Nutz crash the scene with guns blazing. A ferocious battle erupts in the woods and then the city streets as Toxie crashes the party, rapping to throw Budd Berserk, the gang’s volatile leader, off balance. A mop swing becomes a weapon of utmost resolve, and Budd Berserk falls to the chaotic fury unleashed by Toxie.

In a tense turn, Bob Garbinger and his team escalate the conflict by extracting Toxie’s blood to create a serum that could empower Garbinger himself. The plan backfires spectacularly, turning Garbinger into a monstrous predator with horns, claws, and a Krampus-like silhouette. Thad, Spence Barkabus’s father, is killed in the chaos, and the family’s ties become a web of revenge and fear. Wade confronts Fritz, seeking reassurance that his father’s humanity can endure this crisis, and Fritz admits that his family would do the same in his place.

A critical turning point comes when J.J. talks sense into Toxie, reminding him of his humanity and the people he protects. She reveals that his urine is acidic, a strange but practical detail that helps him plan an escape. The two rescue Wade, moving toward the Garbinger compound for a last stand. A gunfight erupts, and a gas-tank blast destroys the shack where Wade was held, leaving Toxie to grieve what he believes is the worst: that Wade might be dead. In the ensuing duel with Monster Garbinger, Kissy—partially mutated herself—swaps sides in a bid for revenge. Toxie’s final challenge becomes a brutal, cinematic showdown: he battles Monster Garbinger, nearly loses, then regains the upper hand with Wade’s courage and Fritz’s crucial choice to help his family escape.

The battle crescendos in a desperate blaze of fire and steel. Kissy lunges with a wooden plank, but a final, cascading chain of explosions devastates the Garbinger compound. Toxie collapses, saved only by Wade’s survival and a poignant exchange: Wade’s belief that Winston is still good, even as the man’s monstrous form persists. Kissy dies in the flames, and Fritz survives, his voice now altered by the chaos but still present in the world.

Recovery arrives at last in the hospital. The doctor who first diagnosed Winston—along with a woman who has endured a separate trauma—confirms that the mutation has cured the tumor.
