In the bustling city of Boston, Irish-American fraternal twins Connor and Murphy MacManus find themselves attending Sunday Mass, where the priest’s heavy remarks about the tragic story of Kitty Genovese ominously foreshadow the chaos that is destined to unfold. As the twins enjoy Saint Patrick’s Day celebrations with friends at a lively pub, their revelry is abruptly interrupted by three Russian mobsters, who are determined to seize control of the establishment for a lucrative redevelopment scheme. Despite the brothers’ attempts to negotiate, a violent confrontation ensues, resulting in a humiliating defeat for the Russians.

The following morning, seeking revenge, two of the vanquished mobsters confront Connor and Murphy, forcing them to act in self-defense, which tragically leads to serious repercussions. As the authorities scramble to piece together the twins’ actions, **FBI Special Agent Paul Smecker is assigned to investigate. Initially hailed as heroes by the press and public, the MacManus twins decide to turn themselves in at a local police station. Smecker conducts a thorough interview, ultimately deciding to let them spend the night in a holding cell to shield them from the surrounding media frenzy.

That life-changing night, the twins sense a divine “calling” that prompts them to embark on a mission to eradicate evil and defend the innocent. As they delve deeper into the murky world of organized crime, they discover that a local hotel is the venue for a clandestine meeting of the Russian mobsters, providing them the perfect opportunity for revenge.

Equipped with weapons acquired from an underground dealer, the brothers launch a surprise attack on the mob, eliminating nine of their foes in a spectacular showdown. Unbeknownst to them, their friend Rocco, an errand boy for the local mafia boss Giuseppe “Papa Joe” Yakavetta, accidentally encounters the mayhem. Faced with the reality of being manipulated by his boss, Rocco recognizes the twins’ cause and chooses to unite with them in their quest for justice.

Their next target is Vincenzo Lapazzi, Papa Joe’s underboss, whom they eliminate during an encounter at a adult theater. However, Rocco’s decisions set off a chain of events leading to Papa Joe hiring the infamous hitman Il Duce to eliminate the twins. When Rocco seeks retribution against his former crew, the trio is ambushed by Il Duce, resulting in severe injuries for Rocco, including the loss of a finger. Nevertheless, they manage to escape, forever altered by their ordeal.

As the police diligently investigate the crime scene, the twins attempt to erase any trace of their actions using ammonia, yet hope dwindles for unraveling the truth. Meanwhile, Detective Smecker, driven by his sharp intellect, uncovers Rocco’s severed finger and meticulously traces the clues back to Rocco and the twins, leaving him in a moral quandary: Should he confront them legally or ally with them, compelled by their seemingly righteous mission? A night out at a gay bar and a profound conversation with a reluctant priest lead Smecker to gravitate towards the duo’s cause.

Later, as Rocco, the twins, and Smecker plot to take down Papa Joe at his lavish mansion, Smecker inadvertently stumbles into a trap. The twins are captured, while Rocco meets a tragic fate at the hands of Papa Joe. As Papa Joe exits, Smecker, disguised as a woman, attempts a rescue but is rendered unconscious by Il Duce.
