Geneticist Nathan Sands and his daughter Nicole are hired by the U.S. Navy to develop a new weapon. They create an intelligent shark with octopus-like tentacles and attach a device on its head to control it via electromagnetic pulses.

During a test mission, S-11 discards the control device, allowing it to operate independently. The creature then heads toward Mexican waters in search of food.

To pursue S-11, Sands and Nicole join forces with fishermen Andy Flynn and Santos and travel down to Mexico aboard Andy's boat. The team works from the sea to track the creature while Sands’s own men follow in a supporting yacht.

S-11 begins killing tourists and locals along the Mexican coast, creating panic and drawing attention from authorities. Sightings become frequent as the creature moves closer to populated areas.

Fisherman Pez sends a photo of the creature to a news station, triggering media coverage. Reporter Stacy Everheart arrives with Bones and begins researching Sands, soon deducing that S-11 is a biological experiment.

Stacy, Bones, and Pez take Pez’s boat into the open sea to gather evidence. They record the creature killing a few people on a beach and prepare to broaden their coverage.

Andy and his diver friends enter a cave to lure S-11 out, but the creature ambushes them and kills all the divers except Andy. The survivors retreat to regroup with Sands’s team.

Following the cave incident, S-11 kills Pez and relentlessly pursues the group. Santos is killed during the ensuing chase as the team closes in on the creature.

Angry at the danger and the orders, Andy radios Sands and declares his intention to kill S-11. Sands’s response proves uncompromising, heightening the tension between the leaders.

S-11 reaches the mainland as Sands and his men arrive and take Andy hostage. Nicole confronts her father about continuing the dangerous experiments, underscoring the ethical line they are crossing.

S-11 intensifies its rampage on the mainland, and Sands’s team is overwhelmed. Sands himself is killed while saving Nicole, marking a tragic turning point in the mission.

Andy and Nicole reunite with Stacy and Bones at a resort under attack by S-11. Stacy and Bones are killed during the assault. Nicole hacks into the brain-chip and uses a device to overload it, while Andy fires at the creature. The overload destroys S-11, and the survivors escape to safety.
